    LIST OF DOCUMENTARY REQUIREMENTS FOR MATERNITY BENEFIT

    REIMBURSEMENT APPLICATION
    BASIC DOCUMENTS DESCRIPTION
    For Live Childbirth – Any ONE of the following, whichever is applicable:
    ▪ Proof of Child’s ▪ Child’s Certificate of Live Birth duly registered with the Local Civil
    Birth Registrar (LCR) or issued by the Philippine Statistics Authority (PSA); or
    ▪ Report of Child’s Birth issued abroad; or
    ▪ Child’s Certificate of Death duly registered with the LCR or issued by the
    PSA or issued abroad; or
    ▪ Child’s passport.
    Note: Documents issued in a foreign country shall be submitted with English
    Translation, if applicable.

    For If Miscarriage
    Miscarriage/Emergency
    Any ONE of the following proofs of pregnancy:
    Termination of
    Pregnancy ▪ Result of pregnancy test duly signed by the physician/municipal health
    (ETP)/Ectopic officer; or
    Pregnancy ▪ Result of other diagnostic test which may include ANY of the following:
    ➢ Ultrasound; or
    ▪ Proof of
    ➢ Blood Pregnancy test (Beta HCG); or
    pregnancy; and
    ➢ Early Pregnancy factor.
    ▪ Proof of AND
    termination of
    Any ONE of the following proofs of termination of pregnancy:
    pregnancy/
    hospital record ▪ Pregnancy test result/s; or
    ▪ Ultrasound result/s; or
    ▪ Histopathological Report.

    If H-Mole/Ectopic Pregnancy
    ▪ Certified true copy of hospital/medical records stating how the condition
    was managed.

    If Still Birth/Fetal Death Any ONE of the following, whichever is applicable:


    ▪ Fetal certificate ▪ Fetal Certificate of Death duly registered with the LCR or issued by the
    of death/hospital/ PSA or issued abroad; or
    medical records
    ▪ Certified true copy of hospital/medical records stating the outcome of
    delivery.
    SUPPORTING DESCRIPTION
    DOCUMENTS
    If Solo Parent (for Any ONE of the following, whichever is applicable:
    delivery date on or after
    ▪ Solo Parent ID issued within two (2) years from date of delivery by the
    11 March 2019 only,
    Local Government Unit (LGU) and signed by the Social Worker and the
    applicable to Live
    City/Municipal Mayor; or
    Childbirth)
    ▪ Certification of eligibility of the Solo Parent issued by the LGU and signed
    ▪ Solo Parent ID
    by the Social Worker/City/ Municipal Mayor, if the ID is not yet available.

    If Caesarian Section If Caesarian Section Delivery (for delivery date prior 11 March 2019 only)
    Delivery (for delivery
    Any ONE of these documents, whichever is applicable:
    date prior 11 March 2019
    only) ▪ Operating Room Record (ORR); or
    ▪ Surgical Memorandum; or
    ▪ Hospital record
    ▪ Discharge Summary Report; or
    indicating type of
    ▪ Medical/Clinical Abstract; or Delivery Report; or
    contingency
    ▪ Detailed invoice showing caesarian delivery charges, for deliveries abroad
    only; or
    ▪ Similar medical documents submitted which indicate the type of
    contingency.

    If Employee is already Please submit the following documents in any of the combinations provided
    separated/AWOL below:
    (without basic
    documents)
    ▪ Proof of
    pregnancy/hospit
    al record;
    ▪ Proof of advance
    payment; and
    ▪ Employer
    Certification.
