Dear Susan Coleman:

Welcome and thank you for choosing the BluePearl Veterinary Partners Charlestown Internal Medicine Service. We
look forward to meeting you and your pet, Max. We love what we do and are committed to providing you and Max with
the best possible care. In order for us to do that, we ask that you make sure your referral information from your primary
care veterinarian is faxed to us at least 24 hours before your first visit. This should include all labwork and xrays that
have been performed within the past few years. In addition, please complete the attached form regarding your pet’s
history and current problems and bring it with you to the initial visit.

What to expect at the first consult?


During the first appointment, Dr. Chartier or Dr. Cavett and the team of nurses will take time to get to know you and
Max. We will discuss many options on how to diagnose and treat Max so that you are comfortable with the plan. Tests
and/or procedures may be performed on this day, but are often scheduled on a separate day. In some cases, we ask
that you leave your pet with us for a period of time during the day. Factors such as cost of the visits, treatments, and
time spent on appointments can be a surprise to someone who is not familiar with a veterinary internist. You should
plan on spending up to 1-2 hours at the hospital for your first consult. The cost of the initial consult does not include
any testing or medications, but you will be given detailed estimates for any treatment plans discussed during your visit
prior to us performing any diagnostics. Please DO NOT feed Max after midnight on the evening prior to your consult in
case we do perform a procedure, unless otherwise instructed when you make your appointment (diabetic patients will
have different instructions). Water can be offered at any time. We always include your primary veterinarian in your
pet’s care plan and all records from our hospital will be sent to Banfield - Stoneham when they are completed.

What to expect after the first consult?


We will always discuss a follow-up plan before you leave your first consult. If you have pending results when you
leave, Dr. Chartier, Dr. Cavett or a nurse will call with the results and plan when the results return. We will usually see
you and Max back in the future as either a recheck appointment with a doctor or as a technician appointment. During a
recheck appointment, you will speak with the doctor to discuss progress and a new plan. These appointments are
usually 30 minutes to an hour. During a technician appointment, you will see a technician only as these are usually
quick appointments for blood work and only scheduled when your pet is feeling well. These appointments are usually
only 15-30 minutes. The doctor or a nurse will call you when the results return.

The Internal Medicine Service is in the hospital Monday through Friday from at least 8am to 5pm and every other
Saturday 8am to 3pm. We are happy to answer any questions that you have during these hours. All messages left
before 3pm will be returned on the same day, but may not be returned until late in the day if we are in appointments
and procedures. If you have an emergency, you can always call or have your pet evaluated through our emergency
service. If you need a medication refill, these can be filled through the Internal Medicine Service during office hours
and require at least 24 hours to be filled. Please do not hesitate to call the office if you have any questions regarding
your appointment.
