Extrajudicial Settlement with

SPECIAL POWER OF ATTORNEY

KNOW ALL MEN BY THESE PRESENTS:

That I, DANILO C. MONDRAGON, Filipino, of legal age, married, a resident of Ichon,


Macrohon, Southern Leyte, Philippines;
Witnesseth:

That I am the only surviving son and sole heir of ADELAIDA C. GALANO, who died in
August 7, 2013 at Ichon, Macrohon, Southern Leyte, Philippines;

That my late mother, ADELAIDA C. GALANO, was a bonafide member of the _______
program of GREEN BANK (EASTWEST BANK) and is entitled to the benefits accorded to her by the
said program;

That relative thereto and pursuant to the pertinent provision of Rule 74 of the Revised Rules of
Court, I do hereby adjudicate the said benefits unto myself being the sole heir of my late mother;

That due to the reason that I could not personally withdraw the said benefits/amount, I do hereby
APPOINT, NAME and CONSTITUTE, ELENA C. BANALO, Filipino, of legal age, married and a
resident of Ichon, Macrohon, Southern Leyte, to be my true and lawful attorney-in-fact, for and in my
name, place and stead for my own use and benefit, to do and perform all or any of the following acts and
things:

1. To withdraw or receive whatever amount/benefits that is/are due to me/us from


EASTWEST BANK, Maasin City Branch, representing proceeds from the aforesaid
program;

2. To deposit the same in my account at LAND BANK of the Philippines, Maasin City
Branch;

3. To execute, sign and authenticate any document/s relevant to or in connection with the
above-stated purpose.

HEREBY GRANTING unto my said attorney-in-fact full power and authority to do and perform
all and every act requisite or necessary to carry into effect the foregoing authority as fully to all intents
and purposes as I might and could do lawfully if personally present and hereby ratifying and confirming
all that my said attorney in fact lawfully do or cause to be done by virtue hereof.

IN WITNESS WHEREOF, I have hereunto set my hand this 27 th day of November, 2013 at
Maasin, Southern Leyte Philippines.
