    AFFIDAVIT OF LOSS

    I, MARY-ANN T. CABANGLAN, of legal age, Filipino, presently


    residing at Aridowen, Sta. Teresita, Cagayan, after being duly sworn, hereby
    depose and state:
    1. That I was issued a Philippine Passport by the Department of Foreign
    Affairs with Passport No. P9390464A valid until November 2, 2028;

    2. That on February 6, 2019 I went at Andalous, Kuwait to work as a


    domestic helper;

    3. That I placed the said passport inside a brown envelop and keep it
    inside a cabinet at the house of my employer at Andalous, Kuwait;

    4. That sometime on December 26, 2020, I looked for the said passport
    inside the envelop but the same could not be found;

    5. That despite all efforts to find the said passport, I still cannot locate it
    and consider it lost;

    6. That I am executing this affidavit to attest to the truth of the foregoing


    facts and to use the same for whatever legal purpose it may serve.

    AFFIDAVIT OF LOSS

    I, MONSIE BIZ DAVALOS, of legal age, Filipino citizen, married, and


    presently residing at Luga, Sta. Teresita, Cagayan, after being duly sworn,
    hereby depose and state:
    1. That I was issued a Philippine Passport by the Department of Foreign
    Affairs sometime on 1992, however, I can no longer recall the Passport
    Number of the said Passport;

    2. That sometime on 1994 I placed the said passport together with other
    documents inside an envelope and keep it inside a cabinet at my above-
    mentioned residence;

    3. That sometime on 2000, I looked for the said passport inside the
    envelope but the same could not be found;

    4. That despite all efforts to find the said passport, I still cannot locate it
    and consider it lost;

    5. That I am executing this affidavit to attest to the truth of the foregoing


    facts and to use the same for whatever legal purpose it may serve.

    7. That I was issued a Philippine Passport by the Department of Foreign
    Affairs with Passport No. P1013702A valid until November 26, 2021;

    8. That I placed the said passport inside a brown envelop;

    9. That on March 19, 2021 I went at Palawan Pawnshop, Sta. Teresita


    Branch;

    10.That I brought with me the brown envelop containing my passport to


    receive the money sent by my sister;

    11.That after receiving the money I went at Sta. Teresita Public Market
    to buy some commodities;

    12.That when I got home, I discovered that the brown envelop


    containing my passport is already missing;
    13.That despite all efforts to find the said passport, I still cannot locate it
    and consider it lost;

    14.That I am executing this affidavit to attest to the truth of the foregoing


    facts and to use the same for whatever legal purpose it may serve.
