ACCOMPLISHMENT REPORT
S.Y 2019-2020

I assisted Junior and senior high school teachers in encoding nutritional status profile of
all learners in QuickNSR tool. I also consolidated, validated and facilitated the early submission of
nutritional status data of the school. I provided technical assistance to 42 class advisers with regards
to the use and need for learner data especially on Nutritional Status report, SF8, mapping of school-
based feeding beneficiaries and retention status of learners. I maintained the effective use of
QuickNSR tool and the reliability of the data. I Kept and stored data in compliance to the Data
Privacy Act. I submitted copies of Nutritional Status Reports to the School heads, School Clinic
Coordinator, School Nutrition Coordinator, School Feeding Coordinator, WINS Coordinator to be
used for their reports. I attended seminars and training workshop on the use of QuickNSR in
Encoding and Consolidation of Nutritional Status Report last June 2019. I represented the school in
Division Roll-out on QuickNSR Tool Updates and Consolidating Nutritional Status Report last
November 21, 2019 I Submitted the Endline nutritional status report for school year 2019-2020 to
district QuickNSR coordinator for district consolidation report.

ACCOMPLISHMENT REPORT
S.Y 2019-2020

I Issued the user accounts to all homeroom advisers both Junior and Senior High School
and give them appropriate access rights to their specific homeroom sections, I ensured that the
homeroom advisers updated all required learners data of Beginning of School Year 2019-2020
(BOSY), I coordinate with the LGU during the conduct of enrollment for the school year 2019-
2020, I updated and submitted all required school data for (BOSY) in the EBEIS, I also ensured
that the basis of the homeroom advisers in enrolling their learners into their section must be the
School Form 1(School Register), I see to it that the homeroom advisers had registered transferees
from private schools and Balik Aral Learners who were enrolled in public schools in the previous
years but not have any LRNs. I provided SF1, SF2, SF5A, SF5B to all SHS advisers and submit
reports to school senior high school registrar for compilation, I attended school meetings and
discussed LIS related issues and concern, I submitted required documents of Learners with
enrollment issues to the Division Planning office. I attended Capacity Building of LIS/ BEIS
school and Truss Coordinators on school data management for BOSY 2019-2020 last August
2019. I represented the school in conferences, LIS trainings and seminars.
