PRE-START UP CHECK LIST


This pre start-up checklist shows items tha must be completed prior to a scheduled start-up. If the answer to
any questions is "NO" (when it should be "YES"), is left blank or contradicts the pump requirements,
the installation is not ready for start-up.

Indicate "N/A" for not applicable whereverappropriate. These question should be answered in conjuction
with the mechanical/piping contractor and electrical contractor. Check list must be returned signed and dated below.

The sole purpose of this pre start-up checklist is to serve as a guide only. LJ Industrial Fabrication, Inc.
does not assume any liability or responsibility for the accuracy of this list or any items omitted. This list does
not absolve the installing contractors for their responsibilitiesfor proper installationin accordance with the
local, nationalk codes, standards and requirements.

Testing and commissioning will be schedule by our service coordinator, upon receipt of complete check list of
requirements and signing company's condition & policy below at least three (3) days.

Company's Condition & Policy


• LJ Insutrial offering as much after sales service support before & / or
during testing commissioning. However, if installation is incomplete or faults develop beyond
our scope of supply, then re-visits to complete commissioning will be charged at the current rate
(Php3,000.00 per day plus airfare & hotel accommodation-customer's account)

GENERAL CHECK LIST: YES NO REMARKS


1.) Pump room is clean and free from any builder's debris,
food, water, etc. allowing reasonable access
2.) Pump room is finished, free from any on-goingconstruction
works, such as welding works, pile works, electrical works,
painting/water proofing works, ets.
3.) Pump room has adequate cooling & ventilation to avoid moisture
4.) Pump room has lighting available
5.) Water Tank is filled with FULL water
6.) Water is clean and free from any solid particles
7.) Permanent power supply available
8.) Voltage power supply constant, normal and within proper range
(does not fluctuate before and / or during and / or after start-up)
9.) Visual Inspection done of the overall pump assembly
(No visible sign of cracks, damage, rust or watermarks)
10.) Entire pump assembly base securely anchored with foundation
bolts of suitable size embedded in the concrete floor or concrete pad
11.) All piping works (system and within pump room) are hydro-tested
Free from any leaks & ready for testing
12.) Entire system has been bleed of all air to avoid water hammer
(For close loop system such as fire protection& booster pump)
13.) All witness personnel's, owners representative, contractors and
Maintenance will be present during actual testing and commissioning
14.) Prepare data settings for the cut in and cut off of Fire Pump
and Jockey Pump
15.) Inlet and Outlet pipes goin in and out the Fire Pump must be
properly supported with braces, bracket and anchored strongly
to resist vibration during activation and functioning of Fire Pump
16.) Put pressure gauge before and after the Main Fire Pump as
provided provision points
17.) Designate representative to whom we will instruct and teach how
to operate the Fire Pump equipment
18.) If the Main Fire Pump is a Vertical Turbine Type prepare 3 liters
of Motor Engine Oil SAE 2000-40 SF/CD.
19.) Sufficient drainage of water
