ACTIVITY 1:

Collage-Making

Directions: Using your cutout pictures, make a collage that will describe your physical, social, and
mental health. Then, answer the follow-up questions on the next page. (3 items x 5 points)
CRITERIA POINTS
Most pictures showed specific pieces of evidence that describe physical, emotional, 5
mental, and social health that are parallel to the student’s explanation.
Some of the pictures showed specific pieces of evidence that describe the student’s 3
physical, emotional, mental, and social health and somewhat parallel to the student’s
explanation.
Pictures showed no piece of evidence that describes the student’s physical, emotional, 1
mental, and social health that are parallel to the student’s explanation.

1. Why is it important to know the factors affecting an individual’s health?


— It is important to know the factors that affects one’s health to keep away from illness
especially now that we have pandemic.
— We all know the saying “Prevention is better than cure” I think this is the best answer to get a
good health because it helps us to avoid those bad habits and foods.

2. Do you think you are healthy? Explain your answer.

— For me, maybe I am because when we are talking about healthy it is a hobby that you've
always doing like exercise,eating healthy foods,doing some personal hygiene etc… When
someone is healthy you need to prove it by testing his/her weakness and observe him/her if
she/he is energetic wherever he/she.

Activity 2:

Direction: Analyze the statement and provide the appropriate answer.

1. The Health Triangle This shows the measure of the different aspects of health: physical, social, and
mental health.
2. Social Health This deal with the way we react with other people within our environment. This may
include public health, family relationships, and peer relationships.
3. Health This refers to the extent of an individual’s continuing physical, emotional, mental, and social
ability to cope with his environment. It is also defined as the measure of our body’s efficiency and over-
all-well being.
4. Cool Down The purpose of this is to slow your level of physical activity gradually, from high intensity
to low intensity.
5. Warm-up This refers to a lower-intensity or lower-speed activity used to prepare or an athletic event or
exercise session.
6. Lifestyle This is the element of physical health that we have the most control over. This includes our
diet, our emotional health, our level of physical activity and our behaviors.
7. Physical This can mean hitting or someone being physically intimidating.
8. Mental Health It deals with how we think, feel and cope with daily life.
9. Healthcare Services These are services that we get to help us prevent illness, detect illness or treat
illness.
10. Physical Activity It is an inclusive term that refers to any expenditure of energy brought about by
bodily movement via the skeletal muscle.
