    On June 29th, 2022, a daily site inspection was performed at the Bogor E-Commerce Hub project. Six unsafe acts and conditions were found, including a pump machine without a protective V-belt cover, workers standing on cable trays without scaffolding, improper use of scaffolding without harnesses, unsecured electrical cables, protruding nails and garbage, and an unsecured gas cylinder. PT. Pulau Intan was required to improve the findings and provide evidence of corrective actions taken.

    Performed daily site inspection, found unsafe
    condition there was no cover/protection installed
    on the V-belt of the pump machine, potentially
    causing worker`s hands to be pinched

    Performed daily site inspection, found unsafe
    action PIBK workers who were installing cables
    stand on cable trays or cable ladders without using
    scaffolding, potentially causing workers to fall
    down and get serious injuries

    verified/tagged (green) by the certified scaffolding
    inspector

    condition a lot of protruding nails and also garbage
    that was not immediately removed

    On June 29th, 2022 time 17:00 PM while BVI-HSE
    Performed daily site inspection, found unsafe
    condition the gas cylinder was left lying on the
    ground without being tied or placed in a
    basket/trolley, potentially causing the gas cylinder
    to explode
