Working at Height
Work in any place where, if there were no precautions in place, a
person could fall a distance liable to cause personal injury e.g. working
on a ladder or a flat roof; falling through a fragile surface.
This also applies in situations that involve a hole, shaft or pit into which
a person could fall such as trenches, pile holes or service pits.

Free Fall

A fall where the fall arrest system experiences
movement in excess of 600mm either vertically or
on a slope which it is not possible to walk on
unassisted before the system begins to take loading

and arrest the fall, the distance of the fall must not
exceed 2 meters.

force to arrest the fall.

Anchor Point
Is a fixture to which a person anchors the line with a hook, to
be held securely and to prevent a fall.
Capable of supporting at least 5,000 pounds (22.2 kN) for
each employee attached
Anchorages used to attach to personal fall protection
equipment must be independent of any anchorage used to
suspend employees or platforms on which employees work.

Self-retracting lifeline/lanyard means a deceleration device containing

a drum-wound line that can be slowly extracted from, or retracted


onto, the drum under slight tension during normal movement by the
employee.
At the onset of a fall, the device automatically locks the drum and

Mobile Fall Arrestors/Rope Grabber


Rope grab means a deceleration device that travels on a lifeline
and automatically, by friction, engages the lifeline and locks so
as to arrest the fall of an employee.
A rope grab usually employs the principle of inertial locking,
cam/lever locking, or both.

A strap with means both for securing about the waist and for attaching
to other components such as a lanyard used with positioning systems,
travel restraint systems, or ladder safety systems.

• If inspection reveals a defective condition, remove the harness


from service immediately and destroy it.
• Inspect labels; all labels should be present and fully legible.
• Inspect stitching; Check for pulled or cut stitches.

Swing Falls
 Swing falls occur when the anchorage point is not directly above the
point where a fall occurs. The force of striking an object in a swing
fall by working as close to the anchorage point as possible.

 Do not permit a swing fall if injury could occur .Swing falls will
significantly increase the clearance required when a self-retracting
lifeline or other variable length connecting subsystem is used.

Personal fall arrest systems


• Limit the maximum arresting force on the employee
to 1,800 pounds (8 kN).
• Personal fall arrest systems are rigged in such a
manner that the employee cannot free fall more than
6 feet (1.8 m) or contact a lower level.

employee to be supported on an elevated vertical
surface, such as a wall or window sill, and work with

• For fixed ladders that extend more than 24 feet (7.3 m)

equipped with a personal fall arrest system, ladder safety
system, and cage.
• Ladder safety systems and their support systems are capable
of withstanding, without failure, a drop test consisting of an
18-inch (41-cm) drop of a 500-pound (227-kg) weight.

Precautions before using ladders, stairs and handrails


 Portable metal or aluminum ladders must be equipped with non-skid
feet.
 Ladders must not be used in a horizontal position as a scaffold.
 Wooden portable ladders must not be painted.
 Personnel must not climb higher than the third rung from the top on
extension ladders or on the second rung on step ladders.
 Aluminum ladders must not be used around electricity.
 The side rails must be held when climbing a fixed ladder.
 Personnel must face a ladder when climbing up or down.

Precautions for using man-riding winch

• Man-riding must be kept to an absolute minimum and shall only

be performed under the strictest of controls and must only take
place under the direct supervision of the Person in charge
(STP/Night Pusher/OIM) of the rig that shall nominate only

trained personnel to perform tasks.


• The approval of the man-riding shall always be with the STP/Night
Pusher unless the rig is assigned to the Tow master, or if the rig is
operating OIM System.
• For Workover Rigs, at night shift Man-riding must only take place
under the direct supervision of the Person in charge who shall be
the Driller.
• Man-riding should be considered a “safety critical” operation.

• A pre-job Safe Job Analysis (SJA) and pre-job safety meeting must
be held.
• A pre-man riding checklist must be completed.
• Specific working instructions must be communicated, understood
and followed by all those involved in the operation.
• A minimum of three people shall be used at all times and clear
lines of communication established.
• Hand signals are to be agreed by all parties prior to

commencement of the man-riding operation.
• Adverse weather conditions and lighting conditions should be
assessed prior to the commencement of any man-riding
operation.

• Where a dedicated man-riding air-hoist is installed, it shall be
used solely for hoisting and lowering personnel

Man-Riding winch must incorporate the following safety features in


their design:
• The hoist-operating lever should automatically return to neutral
on release from any operating position.
• An automatic brake should be fitted so that it will apply whenever
the operating lever is returned to neutral or on loss of power.
• In the event of failure of the automatic brake a secondary brake
should be provided to prevent the load from failing. This may be
manual in operation and simple in design.

• A plate fixed to the frame of the man-riding air hoist stating “For
Man riding Purpose only” and the Safe Working Load should be
clearly identified on the air-hoist.
• A device should be fitted to prevent the
winch from over-riding or under-riding e.g. a
ball type, isolation valve on the air supply
line, close to the hoist-operating handle.
• A functional load-limiting device should be
installed (As practical).

• Anti-spin wire should be used or a nonrotating wire rope sling.
• Hydraulically operated man-riding winches should have self-
locking and self-braking systems fitted.
• The air supply to the hoist will be regulated to the manufacturers

recommended air pressure.
• Spooling device (As practical).

Using man-riding winch


• The employee must use the shackle directly onto the end of the
air hoist line without any intervening chain, slings, swivels or
other fitting. (Shackle must be secured).
• Any tools carried up to the derrick must be tied off to either the
employee or the air hoist line. (All tools should be logged).

• A dedicated, ”tools Aloft” register must be kept on every rig. All


tools carried up to the mast or derrick must be recorded and
signed as having been taken up and then must be signed off as
having been returned to the deck floor on completion of the job.
• The riding harness used shall be of an approved
standard and inspected for wear or damage
before use.
• When man-riding operations are taking place, all
other operations in the vicinity shall be
suspended.

• At no time shall the travelling blocks be moved or
pipe rotated.
• No other activity shall interfere with man-riding

operations.
• Personnel involved in man-riding operations shall
be fully trained and deemed competent to

perform the work.


• When a man-riding-lifting basket is used it must be verified
that the slings and basket have a current Certificate of
Inspection.
• The total weight of the basket, equipment and personnel
must be determined to ensure the safe working load of the
air-hoist and the basket slings are not exceeded.

• Personnel riding in the basket must have a safety line secured to


the air-hoist line and when work is carried out above the monkey
board level, or when it is deemed that verbal and visual
communications are not adequate, radio communication shall be
used and a banksman shall maintain line of sight at all times.
• When any equipment or tools are hoisted
into the derrick, the area below shall be
kept clear of personnel and steps taken to
ensure no one enters into the area.

• Consideration shall be given to the weight
of air-hoist line versus the weight of the man rider when hoisted
above a certain level (e.g. the minimum weight of a man while
using 19mm air-hoist line above the monkey board level is
approximately 200lbs/91kg).

The following are all requirements that you need to consider when
planning and undertaking work at height.

You must:
• Take account of weather conditions that could compromise
worker safety
• Check that the place (e.g. a roof) where work at height is to be
undertaken is safe. Each place where people will work at height
needs to be checked every time, before use

• Stop materials or objects from falling or, if it is not reasonably
practicable to prevent objects falling, take suitable and sufficient
measures to make sure no one can be injured, e.g. use exclusion
zones to keep people away or mesh on scaffold to stop materials
such as tools falling off

• Store materials and objects safely so they won’t cause injury if
they are disturbed or collapse

• Plan for emergencies and rescue, e.g. agree a set procedure for
evacuation
• Think about foreseeable situations and make sure employees
know the emergency procedures.
