Life Jacket, General Inspection 

   Job Description:  Check general condition, cracks and other material defects found requires
replacement of the Lifejacket set with new spare. Check that quantity is correct and that all are in place
according to safety plan. Check all straps are placed as they should, and that they are not entangled or
tied. Check joints are in order and the retro reflective tapes are fixed in place. Check whistle is properly
secured and in working order. Check light on life jacket is in working order and verify expiry date. Wash
and clean if necessary. Check location for and proper stowage where the lifejacket are stowed must be
clearly marked. Life jackets should be stenciled with vessel’s name as a means of
identification. Additional life jackets for large sized person and children stowed in the cabins when
necessary shall be restored to the stowage location on the persons disembarking. Inflatable lifejackets
provided to vessels must be serviced annually. 

 Spare 6 whistles and 6 lights for use with lifejackets must be maintained. There must be one life jacket
per person on board plus an additional amount for persons on watch in the engine control room, the
bridge, the cargo control room and for use at the forward life raft station. A further 4 lifejackets should
be for use by children. Donning instructions for all different types of lifejackets carried on board each
vessel must be clearly displayed. Log entry of report in PMS history file.          Ref. FMS.09.18.1000         

Immersion Suit Inspection    


 

Job Description  Check closure on storage bag as well as general condition of gab for ease removal of
suit. Ensure donning instruction is legible. Confirm that suit is type and identified on the bag. Lay the suit
on a clean, flat surface. Make sure the suit is dry inside and out. Visually check for damage. Rips, tears or
punctures should be repaired in accordance with manufacturer’s instruction by a suitable repair
station* Check the zipper by sliding it up and down to check for ease operation. Using lubricant
recommended by the manufacturer, lubricate the front and back of the zipper and slide fastener. If the
zipper is not functional, the suit should be removed from service and discarded or returned to the
manufacturer or a suitable repair station. If fitted, check inflatable head support and/or buoyancy ring
for damage and ensure that it is properly attached. Check inflation hose(s) for deterioration. At least
quarterly, the head support/buoyancy ring should be inflated and tested for leaks (this test does not
apply to integral inflatable lifejackets). Leaks should be repaired in accordance with manufacturer’s
instruction by a suitable repair station. Check the retro reflective tape for condition and adhesion.
Replace if necessary. Check whistle and expiration date of light and battery. Replace suits in the bags
with zippers fully opened.  Note: Manufacturer`s maintenance manual must be checked for any special
maintenance procedures.  Standard Procedure  The tasks shall be carried out in accordance with original
equipment Manufacturers (OEM) instructions as/where applicable to the equipment installed on board;
Refer to latest service letters and bulletins regarding changes or additions to job description.        
Fire Extinguisher, Inspection    
 

Job Description:  Ensure extinguishers are numbered with a code for ease of identification. Ensure
extinguishers are stamped with pressure test date. Ensure extinguishers are marked with service filling
dates. Ensure extinguishers are located only at their designated positions as per the fire plan. Body of
the portable extinguishers are to be inspected for corrosion. Where corrosion is found in the body the
extinguisher is to be replaced. The pressure relief holes in the screw cap are to be inspected to check
they are clear and the screw threads lightly greased with Vaseline. CO2 cartridges are to be weighed
annually .The CO2 cartridge is to be renewed if its weight falls below 95% of its stamped weight.
Sufficient stocks are to be carried to ensure capacity for 100% refill. Dry Powder extinguishers are to be
inspected annually. This inspection shall include the cap, hose, gaskets and stirring of the powder to
ensure that it is free flowing and weighing of the CO2 cartridge. Where the vessels are fitted with DCP
type, one extinguisher is to be discharged every month during the drill on a rotational basis, so that all
are operated within a stipulated time frame (2 to 4 years).In case of product carriers this may be 2 years
depending on the number of extinguishers on board.  

The vessel should confirm that the retention of the contents when tested is not in excess of 15% of the
initial charge. The CO2 cartridge is to be renewed if its weight falls below 95% of its stamped weight.
Sufficient stocks are to be carried to ensure capacity for 100% refill. Extinguishers are to be check
weighed annually and replaced by a fully charged extinguisher if the weight. Log entry of report in PMS

Air Breathing Set, Inspection    


 

Job Description:  Confirm required sets of SCBA are properly stowed at designated vessel’s fire
stations. The date of test is to be stamped on the air bottle. The air in the bottle is to be fully renewed at
intervals not exceeding 2 years. Each SCBA set must be rigged with fully charged bottles at all
times. When bottles require charging, this must be carried out as soon as is practicable. They should
never be left empty. Face mask, straps, visors and tubing from reducing valve must always be
checked. Facemask and visors must be kept clean at all times. There must always be an equipment list
placed inside the SCBA storage box. SCBA set composed of 3 – main components : Air Supply (Cylinder
and Carrier) Harness (Shoulder Straps and Body Belt) Breathing Assembly ( Face Mask, Air Pressure
Regulator, Supply Tubes, Pressure Gauge, Warning Whistle, Demand Valve) Checkpoints and function
test for SCBA set Cylinder pressure gauge is not defective, no cracks on protective glass, pointer is
indicating the correct volume of air inside the cylinder. Warning whistle for Low Air Pressure Alarm
should sound when the cylinder pressure falls to 40 – 45 bar. This indicates that there is approximately
10-mins of air supply left, this should be considered to be the emergency safe margin for escape. Face
mask should be confirmed air tight when in use, no cracks or deterioration of rubber seal should be
observed, if the seal between the mask and face is less than perfect it is possible that smoke or toxic
gases may leak into the mask. Protect the mask / visor from dust and dirt, always stow facemask and
harness straps fully extended for ease of adjustments. 

 Air cylinder is not corroded, no physical damage, check for air leakage on screwed valve connection,
valve assembly can be operated with ease. Depending on cylinder size, confirm air cylinder fully charged
all the time, as per specifications. Confirm donning and operating instructions for the SCBA outfit is
displayed inside the fire station. Log entry of report in PMS history file.   Note:  Tanker vessels: 4 SCBA
sets with two spare air bottles per set plus one fireman’s outfit per set are to be on board. VLCC vessels:
6 SCBA sets with two spare air bottles per set plus one fireman’s outfit per set.    Ref. CDI 9.1.9 / SOLAS II

Fire Hoses, Pressure Test    


 

Job Description:  The fire and foam mains are to be pressure tested every 12 months to a pressure equal
to at least 10.0 Kg/cm2. Confirm all fire hoses shall be of the approved type by the class society, must
have sufficient length to project a jet of water to any of the spaces in which they are required to be
used. Confirm all fire hoses are provided with approved nozzles, necessary couplings, and special tools
for emergency use. Confirm the following points are always taken into considerations: Hoses should be
protected from contact with paint, solvents and chemicals. Avoid dragging the hose when kinked, do not
kick the hose to free kinks. Protect the hose where it passes over or round sharp edges, where hoses
must pass through doors secure the door open. Open and close hydrants and shut-off nozzles gently to
avoid damage from water hammer. Hot decks must be kept cool to prevent damage to the hose. Under-
run a hose to empty it of water after use, do not walk on it. When stowing hoses. Fold them in two and
roll it from the center. Allow a difference in length between the two couplings, which will be taken up
during the rolling. This way both male and female couplings are presented to the hydrant and the hose
cannot be laid out the wrong way round. In storage the couplings are in the bottom of the locker and
will not chafe the hose due to vibration. Log entry of report in PMS history

A Pyrotechnic, Inspection    
 
Job Description:  Check that quantity of flares and rockets are correct as per inventory list. Every life
boat and life raft must contain pyrotechnics as required by SOLAS. Pyrotechnics shall be replaced in
accordance with the replacement date printed on the packing. This also applies to rockets and igniters
for the line throwing apparatus. All pyrotechnic shall be stored in a waterproof container with label to
indicate the correct contents, check that flares and rockets are stored in dry, marked locations. Check
that the pyrotechnics are located in the marked places, and that they are without damage or
moisture. All pyrotechnics should be marked with an expiry date. Replacement pyrotechnics must be
ordered three months before their expiry date. Expired pyrotechnics should normally be landed against
a receipt at a suitable port, normally the same port at which the replacements are received on
board. Expired pyrotechnics should never be stored on board for any length of time as they present an
identifiable risk and can become unstable, especially if they are hot. This can result in a fire. 

 The list of pyrotechnics in lifeboats shall also be included on the inspections. Every vessel must be fitted
with: One line throwing apparatus (gun type) complete with 4 rocket lines and 4 projectiles (rockets) or
alternatively, 4 self-contained line throwing apparatus. 12 rocket parachute distress flares. 2 self-

Line Throwing Gun (Rocket Driven) Inspection 


 

   Job Descriptions :  Check general condition, cracks and other material defects found requires
replacement of the Line throwing gun set with new spare. Check that quantity is correct and that all are
in place according to safety plan. Check that all guns are in good condition and ready for immediate
use. Check the condition of the line throwing apparatus and the expiry date. Make sure that it is in place
and ready for use. There must be at least 4 rockets and 4 lines available. Check expiry dates of
rockets. Log inspection report in log book and history file…  Safety Inspection Checklist :   Conduct risk
assessment as necessary if safe to proceed taking into consideration accidental discharge of the
equipment.   Communicate to concerned parties regarding the inspection to be performed, duration of
maintenance and procedures to be discussed.   Confirm communication equipments, flashlights, other
safety tools required are ready before the test.   Confirm Line throwing gun safety guards and other
interlocks are not to be bypassed.   Confirm reset procedures are carried out accordingly, designated
personnel to report asap to person in charged for normal conditions of machineries / equipments
affected.   Debriefing to all concerned parties to be performed as necessary for evaluation of the
inspection procedures.   Final test report to be entered into deck and engine logbooks, including related

Escapeways Inspection    
 

Job Description  All escape ways to be checked for proper marking, no obstruction, no blocking, proper
lighting and accessibility.  Standard Procedure  The tasks shall be carried out in accordance with original
equipment Manufacturers (OEM) instructions as/where applicable to the equipment installed on board;

Life/Rescue Boat Davit Routine   


 

Job Description:  All survival craft, rescue boats and launching appliances shall be visually inspected to
ensure that they are ready for use. Grease davits according to the lubrication chart, all furnishings and
movable parts must be in satisfactory conditions. Check oil level in davit winches and refill to the marked
level as necessary, drain water content or replace oil as necessary. Hand and centrifugal brake on davits
must be free of oil and grease. Move and lubricate all movable parts, fill grease through nipples on davit
body, sheaves, winch , wires, and hooks, etc. Check that all furnishing and movable parts are in
satisfactory condition. Release the hook, start the pump, slew the winch both side, lower/raise the hook,
engage the hook to MOB. Slack the Lifeboat falls and free the swivel arrangement. Lubricate with grease
as required. When waterborne: Check release mechanism If applicable the N2 Accumulator system to be
check for correct pressure settings and refilled as per maker recommendations  Log entry of report in
PMS history file.  Standard Procedures  The tasks shall be carried out in accordance with Original
Equipment Manufacturer’s (OEM) instructions as/where applicable to the equipment installed on board.
Refer to latest service letters and bulletins regarding changes or additions to job description.  Safety
Procedures or References  For shutdown of Critical systems and equipment related to planned and
unplanned maintenance it is required as a part of TMSA to involve the office through the technical
superintendent. Please check the Shut Down Procedure in QMS for guidance and approval from
Office. Refer to Maker's safety checklist mentioned in the OEM manual and follow standard QMS safety
procedures.                          Ref.             

Emergency Equipment, General Inspection     


 

Job Descriptions :  Check all items according to the equipment list. Check all emergency escape
breathing apparatus are ready for immediate use. Check face masks and head straps for
rapture/damage Check breathing valve proper operation and functions. Check general appearance of
the bottles. Check bottles are fully charged to the required amount. Carry out function tests of the
apparatus. Check that the filters are of appropriate type according to the cargo currently carried and
that the filters are not expired.  Log inspection report in Log Book and history file…    Notes : Any
apparatus that fails in any of the checks must be subject for maintenance.       
EPIRB (Jotron) Free Float EPIRB, 1M Routine   
 

 Job Descriptions :  Perform internal self test :  Check that the EPIRB’s are located correctly, that they are
without any visible damage and that the operating instruction is intact. Pull out the locking pin on top of
the bracket. Lift the upper arm of the bracket and remove the Tron. Keep the beacon in an upright
position (switch on top of the beacon). Press the spring loaded switch on top of the beacon to the TEST
position. A successful test will consist of a series of blinks on the LED test indicator and internal strobe
light, followed by a continuous light after app.15 seconds. Release the switch and put the beacon back
into the bracket in the inverted position (normal storage position). Replace locking pin on top of the
bracket. Check the bracket for any damages, the beacon should be easily removed and replaced back in
the bracket. Make sure that the bracket is not painted or otherwise covered with chemicals, oil,
etc. Check the expiry date of the hydrostatic release mechanism and the battery unit. Log entry test
report in log book and history file…  Reference :   The EPIRB shall be subject to maintenance at intervals
not exceeding 5-years. It is recommended that the maintenance be performed at the time when the
battery is to be changed. To be performed at an approved shore based maintenance facility.   

FLB, Release Drill    


 

Job Description:  Free fall lifeboats must be free fall launched and maneuvered in the water at least once
every 6 months. However, where possible and in accordance with SOLAS chapter III, regulation 19-3, a
simulated launch can be carried out in lieu of a physical launch strictly in accordance with the
manufacturer’s instructions. Actual free fall test/drill is recommended to be performed at the same time
during the maker’s annual service inspection. All personnel involved should be fully trained in all
operational modes of the boat and it’s launching appliances. Failure to follow the proper procedures for
launching may result in death or serious injury. The free fall type life boat can also be launched by using
davit fall wire to lower and recover the boat to and from the water, as applicable. Log entry of report in
PMS history file.   Boarding the lifeboat: Upon mastering personnel should enter the lifeboat quickly and
orderly carrying their lifejackets/survival suit. Boarding should be done evenly for both side seating
arrangement. The hook release operator should board last after doing the pre-launch checks. Once
seated, secure the safety harness and tighten adjustable lap straps. Place knees against the seat in front
and push body firmly against the backrest of your own seat, keep head facing aft and do not turn to
sides.  Free Fall Launching Mode – Normal: Check the lowering / retrieval hooks have been disconnected
from the lifeboat lifting slings and the slings are stowed on top of the boat.  

The boat shall always have these slings disconnected and ready for immediate use in an
emergency. Lashing arrangements has been disconnected, the boat is lashed with a bolt in the aft, all
ropes, etc. must be clear of the lifeboat and launching ramp. Electrical plug for battery charging outside
of the lifeboat has been disconnected. All hatches and openings are securely closed and locked, the
helmsman must confirm that landing area is clear. Switch “On” the battery and start the engine in
neutral position. Helmsman must advise the start for the hydraulic pumping release procedures, after a
few strokes, pressure should be felt in the pump handle as the boat shall start to lift until it release itself
by free fall into the water. When the boat is waterborne turn the knob of the hydraulic pump to allow
the hydraulic cylinder below to retract. The helmsman should engage the propeller and steers the boat
away from the vessel. All personnel shall remain seated unless instructed otherwise by the officer in
charged. Should the normal free fall launching fails the emergency launching procedure must be done
using the back-up secondary hydraulic pump arrangement. 

Life raft monthly Inspection.    


 
Job Descriptions :  Check lashings, painter and general condition of the container. Check the hydrostatic
release and lubricate as necessary. Check that markings of container are clearly visible. Check that
launching procedure poster is posted close to launching area. Check drain holes are turned downwards
and not clogged. Check embarkation ladder, rope, steps, shackles, furnishing etc., renew defective parts.
Check life raft cradle, clean and re-paint rusted sections as required. Log entry of inspection report in
history file…   NOTE: In areas where icing can occurred, HRU to be kept free of ice…   Safety Inspection
Checklist :   Conduct risk assessment as necessary if safe to proceed taking into consideration vessel
operations.   Communicate to all concerned parties regarding the inspection to be performed, duration
of maintenance and procedures to be discussed.   Confirm communication equipments, flashlights,
other safety tools required are ready before the test.   Confirm Life raft installation safety guards and
other interlocks are not to be bypassed.   Confirm reset procedures are carried out accordingly,
designated personnel to report asap to person in charged for normal conditions of machineries /
equipments affected.   Debriefing to all concerned parties to be performed as necessary for evaluation
of the inspection procedures.   Final test report to be entered into deck and engine logbooks, including

Firemans Outfit monthly inspection     


 

Job Descriptions :  Confirm required sets of fireman’s outfit properly stowed at designated vessel’s fire
stations, for reference consult vessel’s fire control and safety equipment plan. Fireman’s outfit shall
contain the following items as minimum requirements :  Qty Item Descriptions Remarks  1 set Self-

Spare lamp is attached inside the casing.  1 set Fire Axe with belt   1 set Fire resistant Safety Line with

Checkpoints for Fire Protective Suit : The suit should ideally be stowed with trousers over the boots so
that the wearer can step into the boots and pull up the trousers, this cuts down the time taken to dress
the fire fighters. Fire protective suits are made of lightweight water resistant and flamefroof materials,
confirm protective layers are intact, no flaking or ruptures should be found. Confirm donning and
operating instructions for the complete Fireman’s outfit is displayed inside the fire station. Confirm
spare fully charged air bottles are properly stowed on every fire stations.         Safety Inspection Checklist

equipment for BA equipment.   Communicate to all concerned parties regarding the inspection to be
performed, duration of maintenance and procedures to be discussed.   Confirm communication

safety guards and other interlocks are not to be bypassed.   Confirm reset procedures are carried out
accordingly, designated personnel to report asap to person in charged for normal conditions of
machineries / equipments affected.   Debriefing to all concerned parties to be performed as necessary
for evaluation of the inspection procedures.   Final test report to be entered into deck and engine

Fall Protection Eqpt/Safety Harness, 1W Routine 


 

   Job Descriptions :   Storage area, routine check: Clean, tidy and dry storage area. Floor tidy and
clean. No storage of others that can damage the safety harness.  Safety harness, routine check for: Any
visible damage on the equipment. Equipment properly stored and well arranged. Defect equipment to
be tagged out and isolated. New equipment to be ordered   

SAF0105A Rescue Boat, Launch Drill    


 

Job Description:  All personnel involved should be fully trained in all operational modes of the boat and
it’s launching appliances, failure to follow the proper procedures for launching may result in death or
serious injury Fast rescue boats and launching appliances shall be visually inspected to ensure that they
are ready for use.  The inspection shall include, but not be limited to, the condition of hooks, their
attachment to the lifeboat, and the on-load release gear being properly and completely reset.  Inspect
the davits and ensure that they are ready for immediate use. Standby rescue boat crew and launching
party, test communication equipment. Swing out rescue boat to the embarkation deck upon order from
the Master. Once subject / person is sighted, stop the vessel to windward of the person, lower and
release the rescue boat by on load release operations. Log entry of report in PMS history

SART, 1M Routine 
 

   Job Descriptions :  Monthly Inspection and maintenance for SART 9GHz radar transponder. Keep SART
and container as clean as possible. The SART and container should be inspected and rinsed off
periodically Confirm that no debris such as rags of other materials is stored on or around the SART
assembly.  Confirm that no new structures or obstacles have been installed in the area of the SART that
might inhibit its carrying Confirm that the SART transponders are located correctly, that they are without
any visible damage and that the quick operation guide is intact. Check the mounting hardware for
tightness if it is used. Inspect the SART and the container for any signs of deterioration, cracks or other
damages. Check the battery expiry date shown on the SART. If the battery expiry date is indicated,
contact your nearest JRC dealer to have the battery replaced. At a electrical shielded room of the ship ,
activate the SART by removing the tail-cap ,and turn the switch to "ST-BY" and check that the green LED
will be illuminated.(stand-by condition) After checking stand-by condition, turn off the switch and check
that the green LED is not illuminated, and fasten the tail-cap tightly until the lock marks are aligned.

Lifeboat & Launching Appl, 60M Routine   


 

Job Descriptions :  Refer to IMO Circular letter 1093 of 17 June 2003 New regulation apply to all ships
from 1st July 2006  In addition to the 12 monthly Service (ROU180) testing off the winch brake should be
tested with the boat loaded to a proof load of 1.1 times the maximum working load of the winch. For
lifeboats with fall preventer, the preventer shall be disconnected and tested to 1.1 times the max
working load. Inspection and thorough examination of lifeboats, launching appliances and on-load
release gear, to be carried out of manufacturer representatives or competent companies as described in
the circular letter and in accordance with instruction book. Operational testing of winch brake should be
carried out with a mass equal to lifeboat without persons onboard. The dynamic brake test constitutes
the five yearly operational testing which should be done by lowering the the boat loaded to a proof load
equal to 1.1 time the weight of the survival craft or rescue boat and it’s full complement of persons and
equipment, or for safety reason an equivalent load. When the boat has reached it’s maximum lowering
speed and before the boat enters the water, the brake should be abruptly applied. In loading the
lifeboat for this test, precautions should be taken to ensure that the stability of the boat is not adversely
affected by free surface effects or the raising of the centre gravity.   The 5 - years service requirements
are similar to the annual service which applies on :  Lifeboat Condition of lifeboat structure including
fixed and loose equipments Engine and propulsion system Sprinkler system, as fitted Air supply system,
as fitted Manoeuvring system Power supply system Bailing system       Release Gear Operation of devices
for activation of release gear Excessive free play / tolerances Hydrostatic interlock system, as
fitted Cables for control and release Hook fastening Notes :   The setting and maintenance of release
gear are critical operations with regard to maintaining the safe operation of the lifeboat and the safety
of personnel in the lifeboat. All inspection and maintenance operations on this equipment should
therefore be carried out with outmost care. No maintenance or adjustment of the release gear should
be undertaken while the hooks are under load. Hanging off pennants may be used for this purpose but
should not be remain connected at other times, such as when lifeboats are normally stowed and during
training exercises. 

  For reference, operational test of on-load release function : Position the lifeboat not more than 1-
meter into the water, such that the mass of the boat is substantially supported by the falls and the
hydrostatic interlock system is not triggered. Operate the on-load release mechanism. Reset the on-load
release mechanism with caution as the boat is water borne. Prior to hoisting, check that the release gear
is completely and properly reset, no person shall be allowed to stay during hoisting of the
lifeboat. Examine the release gear and hook fastening to ensure that the hook is completely reset and
no damage has occurred. Recover the lifeboat to the stowed position and prepare for operational
readiness.  For reference, operational test off-load release function : Position the lifeboat fully
waterborne. Operate the off-load release gear, confirm the hooks are simultaneously released. Reset
the off-load release gear mechanism with caution. Prior to hoisting, check that the release gear is
completely and properly reset, no person shall be allowed to stay during hoisting of the
lifeboat. Recover the lifeboat to the stowed position and prepare for operational readiness.    For
reference, operational test of Free-fall lifeboat release function (simulated launching) : Engage the
simulated launching arrangements as specified in the manufacturer’s operating instructions. The
lifeboat release pin shall be released while the lifeboat is still secured / strapped into the cradle.  The
operator should be properly seated and secured according to the lifeboat operating
instructions. Operate the release mechanism according to recommended instructions, the lifeboat shall
move just a few inches while still lashed as the holding pin retracts back. Reset the lifeboat in stowed
configuration, repeat the procedures using the back up release mechanism, as applicable. Remove the
simulated launching arrangements and verify that the lifeboat is ready for launch in stowed

letter 13/98, measure centrifugal brake shoe thickness before the 1st and after the 5th lowering. Record
the wears. (Average of 4 points on shoe). Confirm the lifeboat is secured from accidental launching,
open and inspect the brake mechanism assembly, mark relevant points before dis-assembly. Inspect
brake internal parts, pads, springs, and other parts prone to wears and corrosion, clean or replace brake
pads as required. Check conditions of electrical and mechanical remote control system, grease-up wires,
rollers, pivots, and other parts prone to sticking due to corrosions, paint, and other obstructions.  Check
conditions and functions of stored power system, electrical control panel.  Check conditions of winch el-
motor and foundation base for abnormalities. Operational testing should preferably be done by
lowering the empty lifeboat, when it reached it’s maximum lowering speed and before the boat is water
born the brake should be abruptly applied to check it’s effectiveness.  Following this test, the brake pads
and stressed structural parts should be re-inspected for abnormalities.  The inspection records /
checklist for weekly, monthly, and annual inspections also forms the first part of the thorough
examination, when carrying out this inspections by the ship’s crew it must be with the presence of the
manufacturer’s representative or third party certified by the maker to conduct the inspections.  All
repairs and replacement of parts should be carried out in accordance with manufacturer’s requirements
and standards.  The 5-years maintenance to be carried out with the presence of Chief Engineer and
Surveyor  Standard Procedures  The tasks shall be carried out in accordance with Original Equipment
Manufacturer’s (OEM) instructions as/where applicable to the equipment installed on board.
