That I have never been approached by any employee of the Philippine Overseas Employment Administration (POEA), or of Industrial Personnel and Management Services Inc. (IPAMS), and asked to pay any Placement Fee or make any contribution for the services rendered to me in obtaining a job overseas as _____________________________ with MH ALSHAYA CO. W.L.L. / DAREEN INTERNATIONAL CO. (QATAR)
2. That I did not make any payment or contribution in whatever form to any person acting as recruiter or agent of IPAMS, or in any other capacity for services rendered to me in my application for the above-mentioned job overseas;
3. That, in the event I withdraw my application while processing is in progress, I agree to reimburse IPAMS the actual and administrative costs pertaining to my application for employment as per attached “Offer of Employment” and “Schedule of Administrative Charges for Withdrawing Candidate” forms I signed; (Annex A)
4. That I have not been involved, charged or have committed any offense, criminal, civil or administrative, nor have been apprehended by police authorities, or NBI in the Philippines, or equivalent authorities in foreign countries, nor have been incarcerated, for reason I may be refused entry to start employment in the country where I will be deployed;
5. That I did not provide or submit false information or documents for purposes of this job application which is an offense and a ground for disciplinary action against Overseas Workers under Part VII Rule III of 2002 POEA Rules and Regulations governing the Recruitment and Employment of Land-based Overseas Workers as Revised in 2016.
6. That I do not have any pending case involving monetary accountabilities with my former employers in the Philippines and in foreign countries where I have worked before and I am not under the POEA watchlist, neither have a pending case at the POEA or the National Labor Relations Commission (NLRC);
7. That in the event I am denied visa / refused entry into the country where I will be employed, for misrepresentation / non-disclosure of significant events regarding my previous local or overseas employment, or any reason of untruthfulness of the foregoing statement, I will not hold IPAMS and/or MH ALSHAYA CO. W.L.L. / DAREEN INTERNATIONAL CO. (QATAR) liable for my repatriation, and that I shall reimburse and pay all costs incurred by IPAMS and/or MH ALSHAYA CO. W.L.L. / DAREEN INTERNATIONAL CO. (QATAR) for my mobilization and repatriation, including applicable damages and penalties as may be imposed by the Principal/Employer, and will not file any complaint or claim whatsoever as a result thereof;
8. That (for married candidates), my spouse is fully aware and has personal knowledge that I have signed an Offer of Employment and “Schedule of Administrative Charges for Withdrawing Candidate” as mentioned above in Item 3, and that he / she has no objection whatsoever to my joining the company for overseas employment;
9. That I am voluntarily and willfully executing this Affidavit, free from duress, threat or intimidation.
