    AFFIDAVIT OF EXPLANATION

    I, Ruben B. Mila Jr., of legal age, single, Filipino, resident of


    Brgy. Calanipawan, Tacloban City, member of the Philippine National
    Police with the rank of Patrolman, assigned at Ormoc City Mobile Force
    Company, Camp Downes, Ormoc City and detailed service at Regional
    Public Information Office, Camp Ruperto K. Kangleon, Brgy. Campetic,
    Palo, Leyte, as VIDEO EDITOR and TOASTMASTER, after having been
    duly sworn to in accordance with law, hereby depose and say:

    1. That, in accordance with Memorandum Order dated October


    5, 2022 issued by Ormoc City Police Office, signed by OIC,
    CARMU, ordering for an explanation from the affiant for his
    failure to submit and update his Declared Legal Beneficiaries
    (DLB) on September 22, 2022;

    2. That the affiant’s failure to submit and update his Declared


    Legal Beneficiaries (DLB) on September 22, 2022 was a mere
    inadvertence, an overlooked, and unintentional in nature;

    3. That the affiant’s tasks in the Regional Office, as the official


    VIDEO EDITOR of the Regional Director’s presentations in
    Camp Crame or Area Police Command-Visayas, and
    sometimes, if not often, by the Command Group, Regional
    Staffs, and Regional Support Units, requires all the time of
    the affiant to complete and most of the time, the affiant,
    brings his unfinished tasks home to satisfy such until wee
    hours;

    4. That the affiant is also the official TOASTMASTER of Major


    Events and some non-major events in the regional office;

    5. That, due to this overwhelming tasks, the affiant might have


    forgotten inadvertently, with no purpose of evasion or ill-will
    of defying a lawful order from his superiors, from complying
    with such directive issued on September 22, 2022;

    6. In line with the foregoing, the affiant, after having been


    notified of this inadvertence, immediately went to the
    Regional Personnel and Records Management Division (R1) to
    inquire and ask for the list of the requirements in updating his
    Declared Legal Beneficiaries (DLB);

    7. That the affiant executes this affidavit of explanation to attest


    to the truth of the foregoing facts and promises to comply
    with such order this coming Friday or within next week.
