ACCOMPLISHMENT REPORT
PROJECT TITLE: I-ENROL MO, TUTUTUKAN KO!
SY 2021-2022

TARGET LEARNERS: Learners with problems with regards to LIS.

OBJECTIVE: The Learners' Information System (LIS), an online tool for registering
students enrolled in public schools, was launched by the Department of
Education in 2011. Since its implementation, the LIS has enabled the
Department to calculate total public-school enrolment based on actual learner
registration.
This project will propel the school LIS to become a cutting-edge instrument
for managing information with the goal of encouraging transparency,
informed decision-making, and empowerment at all levels of the organization.
While the LIS is a technological solution, it is also a community-driven
process that survives thanks to the active engagement and participation of all
teachers, principals, planning officers, and other DepEd workers across the
Philippines.

RESULTS: Zero – Out the pending learners

ACCOMPLISHMENTS: Any nation would want to create and maintain a uniform vision for its
educational system: one that supports cultural diversity, is free of prejudice,
provides a secure environment and upholds human rights, and develops
qualities, abilities, and talent among others.
“The State shall protect and promote the right of all citizens to quality
education at all levels and shall take appropriate steps to make education
accessible to all.”- 1987 Constitution.
Learners must develop the attitude that learning is not a given. Having
access to education should be treasured, especially during times of crisis and
abrupt change. In addition to being a fundamental human right, it also affects
the other rights we enjoy. Furthermore, according to the UN, maintaining a
sustained state will be extremely unlikely when learning mechanisms fail.
Project accomplishment highlights the activities done by the school to keep
up with the learners management system activities since there is a continuous
increase in enrolment for 5 consecutive years.
These activities started with teachers reorientation on facilitation of
enrolment once the sections are created. It is followed by the provision of
technical assistance for enrolling learners with problems such as checking

correct information based on learners' live birth certificate and entry of address
and others.
In addition, this project also opens the line of communication among teachers
within school and outside the school, since we have increasing enrolment the
LIS school coordinator obliged class advisers to call for parents and previous
school for the correction we have found out about the learner’s data.
Lastly and most importantly, because of this project we were able to lower
the number of pending learners in the 1st quarter from 32 learners to 7 pending
learners and zero-out pending learners in the least quarter.

WAYS FORWARD: Continuing the project and adding other training relative to Learners data
management is recommended.

ACCOMPLISHMENT REPORT
PROJECT TITLE: MAG-AARAL KO, TUTULUNGAN KO

TARGET LEARNERS: 85%

OBJECTIVE: To determine and keep track the performance of learners


To find partners in the community or different organizations that may help the school
especially the pupils in this time of pandemic
RESULTS: Effective-reached 76% accomplishments
ACCOMPLISHMENTS: Teachers conducted home visitations to give us a glimpse into the life of our students
and their families on a more personal level. We can connect and converse with the
families, especially those who don’t usually visit the school. We can see for ourselves
the situation our learners are experiencing. Through this, we get to understand more and
develop empathy toward our students. By conducting home visitation, we monitored the
performance of our learners in different learning areas.
Teachers sent educational videos to their learners thru GC or FB page.
Teachers look for stakeholders from the community to provide our learners with more
resources for school improvement and pupils’ needs.
Learners are encouraged to attend Kamustahan once a week

PROJECT TITLE: HATID DUNONG SA TAHANAN, IPASA MO
(Brigada Pagbasa – Project that will reach each home)
TARGET LEARNERS: 35
OBJECTIVE: To strengthen the communication between the learners and the teachers or vice-
versa, to enrich the literacy skills through home visitation and creating reading
corners, and to provide the needs of the students amidst the pandemic.
RESULTS: Effective reached 10% accomplishment
ACCOMPLISHMENTS This program helps our learners to improve their literacy skills. It also lightens
: the interest of the learners in reading by creating reading corners in some
designated places or home wherein there are several books that are available to
read. This program was a big help to all the teachers, especially at primary level
to determine who among them are considered as nonreader, dependent,
instructional and independent readers.

ACCOMPLISHMENT REPORT ON KUMUSTAHANAN


OBJECTIVES:
� Provide a venue for people to express and thoughts as they are affected by the
pandemic.

� Help individuals with their mental health issues and be able to function normally and
recover from coping optimally everyday.

� Suggest individuals to prevent their distress and suffering to progress into a severe
state.

� Assist individuals to get access to basic services ( i.e information assistance. etc.)

� Assist individuals to obtain a higher level of mental health services, when necessary.

Results
� Online Kumustahan is effective because 50% of the pupils learned through follow up
chat, GC and youtube.

ACCOMPLISHMENTS
� Teachers with a significant meaningful experience in face to face classes in a school with
an animating presence at home become a strong foundation to make education happen
more meaningfully in an online environment.

� The situation may be different. But the commitment of teachers and the philosophy of
education of the school are both helpful whether the learning environment is physical
or virtual.

WAYS FORWARD
� Monitor and update learners’ will be continued for the next quarter and for the
expanded face to face classes by the help of the teachers, parents , Barangay and LGU’s

ACCOMPLISHMENT REPORT
PROJECT TITLE: Project SK (Support Kami)
TARGET LEARNERS: 80% Of projected learners
OBJECTIVE: To enhance the learners’ quality of learning and meet the learning style they need.
To collaborate with the Sangguniang Kabataan and Volunteer teachers to increase
the development of the learners.  

RESULTS: Effective-reached 70% accomplishments


ACCOMPLISHMENTS Sangguniang Kabataan and teacher-volunteers conducted “Sabado-Kumustahan
: para sa Kabataang Laguerta” which aims to engage the learners in education. This
project assisted the learners to improve knowledge about previous lessons.
“Sabado Kumustahan” conducted once a week with the collaboration of the
Laguerta ES Teachers, SK, and Teacher-volunteers of the community. Learners
are monitored by their homeroom teachers through checking their participation and
attendance in the event. While SK and teacher-volunteers of the community are
assigned to perform the task of reviewing the learners’ lesson. With this project it
creates learning opportunities for teachers, learners and other stakeholders to
produce a harmonious environment which learners can benefit.
WAYS FORWARD: Laguerta Teachers and Sangguniang Kabataan collaboration will be continued for
this next coming school year to monitor learners’ quality of learning through this
project.

ACCOMPLISHMENT REPORT
PROJECT TITLE: KAPE’T GURO
SY 2021-2022
TARGET LEARNERS: 18 teachers
OBJECTIVE: Improve teacher’s competence in their stated development goals on the IPDP
based on E-SAT Result through conducting LAC Sessions and other relative
coaching and mentoring activities.
RESULTS: Mentors and Coach teachers on their needs for development through the
conduct of LAC sessions and during coffee time.
Completed the LAC Plan in time with 2 additional topics as needed by the
teachers.
ACCOMPLISHMENTS: For learners to achieve, they need to have a competent teacher. So how does
one become a great teacher? Teachers must undergo training, just like any other
specialized job. Prior to entering the classroom, students must get training, and
they must continue to do so while inside. Throughout their careers, instructors
receive continued training, which can range from student teaching to ongoing
professional development (PD).
With all of this preparation, established teachers are supported as they take
on new challenges in the classroom and novice teachers are given the best
possible chance for success. Teachers run the danger of quitting their jobs too
soon if this training is not provided. Another worry is that pupils may suffer
from inadequate training.
This project follows the LAC plan prepared by the school. What makes this
KAPE’T GURO project special is it gives additional input to the teacher based
on what is present with the situation. There are partner teachers in each topic
and they will talk about it, mostly teacher mentor and teacher mentee (Mentor
and those teachers under TIP).
Coffee time, snack time and even during lunches, this program takes its
place. Conversing on how to deal with a learner in a case. Teachers' attitude and
being ethical outside the school and being professional are some of the inside
topics we have inserted in the LAC. Uplifting teachers' morals through telling
stories help each other to become more “PROFESSIONAL TEACHER LIKE”.
Aside from the in-time LAC Session on plan, we have included Code of
Ethics of a teacher as an additional topic, which dealt with how we can be better
people not generally because we are teachers but because we are morally
upright people.
We can better serve our learners if we have an avenue to express our
experiences, our ideas and thoughts on a case or problem. Through this project,

teachers feel easy but yet focused on their job. Employees are more present and
more likely to focus on their jobs when they work in a joyful environment
because they are happier and healthier. That is supported by different
achievements of the school connected to teachers who are happy working.
KAMI ang nagiging karugtong na Kamay ng aming Kasamahang Guro.
Kapit lang.

ACCOMPLISHMENT REPORT
PROJECT TITLE: Project Pulsuhan Mo, Aaksyunan Ko

TARGET LEARNERS: 34% Struggling and Emergent Learners, Parents and Struggling
Learning Partners

OBJECTIVE: To create activities and related studies as a responding mechanism


in answering school issues that are existing based from the
surveys, feedbacks and requests coming from all walks of the
school community.
RESULTS: Effective-reached 25%% accomplishments
ACCOMPLISHMENTS: In order to make necessary actions from the parents and other
stakeholders’ feedback, the school created two studies entitled
Project REACH which is a continuation of study last S.Y. 2020-
2021 that aims to give importance on teaching children with
materials found at home for contextualized learning. We also
created another study entitled Project DELTA that aims to help
learning partners to better aid their learners at home by providing
them with some webinars and simple training that could better
equip the parents to teach reading to our emergent and struggling
readers from Key Stage 1. We also prepared a Brigada Pagbasa
Program to address the challenges that schools are facing today,
specifically this time of pandemic is to reach out each learner in
any reading level to become readers, especially those who are
under frustration in reading level.
WAYS FORWARD: We still have to continue the studies and sustain the gathering of
feedback from all members of the school community to better
evaluate the school performance and take some necessary action
to improve and constructively process each gathered information
for future school improvement projects.
