MODULE 1
Lesson 1: Community Health Process

Reflective Thinking Activity


1. Think of the actions/procedures you did during your duties especially in the
community. Which among these procedures have you performed assessment?
- When we experienced having duties in the community health unit, we performed
getting the fetal heart rate of the baby using a doppler (mostly stethoscope),
visiting some patients, checking their vital signs, and assisting women with
breastfeeding and infant care. There are lots of actions we did and checking vital
signs including getting FHT of the body through the pregnant woman’s womb are
examples of performing an assessment.

2. What are your experience while doing assessment to your patients?


- At first, it is kind of nervous like there are butterflies in my stomach because I’ve
been thinking what if I did it wrong or what if I’ve missed something in
performing assessments on my patients. Every time I’m doing and continuing a
task like this, it gives my heart pounding and some relaxation that I made it
successful while taking care of them.

3. What makes assessment crucial? Explain.


- Assessment is a tool to learn about the patient’s concerns, symptoms, and overall
health. During the assessment, midwives may notice signs of potential or
underlying health issues that need to be addressed, as well. It is very crucial even
though it is the first step. Yet, assessment plays a decisive role in determining the
patient’s health problems and caring needs. This is very important, especially in
gathering data and information because it depends on the whole process of the
nursing care plan when there are wrong details on it. So being a student midwife, I
suggest assessing them carefully and to those patient’s sides make sure you are
telling the truth when they are assessing you because it can affect you seriously.

1. Why do community health process important to midwives?
- We say that midwives are the one who assists the mother during childbirth and
primarily maternity care. This community health process helps to diagnose the
client’s health care requirements, define priorities, set objectives and expected
results of treatment, create and communicate a client-centered treatment plan, and
conduct midwife interventions to fulfill client objectives and goals.

2. If you are to assess your community, what would be your initial action? Explain.
- It would be the collection of data because data are collected systematically and
continuously that are recorded in appropriate forms. It easily identifies factors that
can cause patients to be influenced by death and illness.
3. In a site selection for the conduct of community health process, what are the criteria’s
to be considered?

5. Do you need to document all the actions you did to your patient? Why?
- Yes, because it I important to document all the data on what, why, and how
clinical care is delivered to patients because sometimes there is a sudden failing
action to patients. These records help other practitioners to understand the
patient’s history so that they can continue to provide the best care feasible for each
patient.

6. Why do you need to follow these steps/process in the community health?


- Yes, it is very important to follow these steps because it can be your basis as a
midwife to treat your patient successfully/ you can also determine some factors
which could trigger him/her on the illness/diseases he/she has community health
process is a very effective tool to help people solve their problems in health and
meet their health and nursing needs.

7. Why do evaluation of the implemented actions significant to your future profession?

8. Identify a one priority problem each category (health threat, health deficit, foreseeable
crisis) in your community. Make a plan of action following these steps: identified
problems, goal setting, plan of actions, evaluation.

MODULE 2
Lesson 1: Prenatal

1. As a future midwife, why do we need to perform prenatal check up to pregnant
woman?
- It is very important to have a prenatal check-up for pregnant women because it
can help prevent complications and inform women about important steps, they can
take to protect their infant and ensure their healthy pregnancy. It is the only thing
that pregnant women must have a regular prenatal checkup to reduce the risk of
any complications during their pregnancy.

2. A pregnant came and you perform prenatal, some of the procedures you are doing
is against her tradition. How do you deal with it?

3. Make a video clip of yourself performing prenatal checkup. Choose one member
of your family to act as your patient, preferably a girl.
MODULE 2
Lesson 2: Immunization

1. How can you help your community in the reduction of life-threatening diseases?
2. Why do you need to maintain the cold chain system?
3. A patient with a 24-week baby came to the center. You know that you have increasing
cases of measles? She asks you if her baby can have a measles vaccination, what will
you do? Why?
4. There is a scheduled immunization in your barangay, and they ask you to assist. You
encountered a first patient who wants her baby to be vaccinated with BCG, you
checked the records and her baby is only the one who will be vaccinated that day.
What will you do? Explain your answer.
5. A patient came with a baby who is experiencing colds and she asks you if her baby
can receive the immunization. What will be your answer? Why?

MODULE 2:
Lesson 5: Mother’s Class

Let’s See What You Have Learned


1. All the programs/activities that you’ve read have a common goal. As a student
midwife, what will be your contribution to attain that goals and objectives?
2. The government are doing their part in helping health programs and yet there is still
increasing rate of maternal and child mortality and morbidity. What do you think are
the factors of not attaining the goal of the DOH in terms of maternal and child
programs? Why?
3. Among the clinic activities, which do you think is suitable procedure/activities to be
performed to pre-schoolers? Explain.
4. As a future midwife, how important IMCI is? Cite one scenario when do you apply or
perform this?
5. One pregnant patient came to you for a prenatal check-up, as an initial action you
have to assess your patient. She told you that her LMP was last June 19, 2020. How
many weeks is she now? When will be her expected date of delivery?
6. You are invited to educate mothers in your community. If you are to decide your topic
doing that class, what would it be? Why?
7. Choose one among the different immunization except OPV and perform using a
dummy. Record it using video.

Lesson 1: Bag Technique

1. One of the functions of community midwife is to conduct home visit. Why do you
need to set your objective or goal first before doing home visit?
2. How important documentation during home visit? Why do we include midwife-
patient relationship during home visit?
3. Recode a video while doing these:
- Bring out your public health bag
- Check if it is complete basing on the contents written on Lesson 1.
- Arrange the ocntents.
- Perform a home visit (conduct it in your house) following the steps until
evaluation.

1. Postpartum home visit involves the mother and her child. If you are to visit a mother
who had home delivery in your community, how will you explain the importance of
the postpartum visit to a reluctant mother?
2. If you encounter a postpartum mother who told you that her 6 days old baby has foul
smelly umbilical cord, what would be your action as a student midwife?
3. Assuming you are already a midwife assigned in a far-flung community, you
encountered a pregnant mother who will deliver her baby and you do not have your
own birthing facility. Upon IE, the patient is already 10 cm. Are you going to deliver
at home or travel it to the nearest facility? Support your answer.
4. How will you convince a mother/pregnant to give birth in a birthing facility?
