Instruction

TRANSFERRING CLIENT

MATERIALS/EQUIPMENT:

1. Transfer forms
2. Wheelchair / stretcher (optional)
3. Oxygen appliance (optional)

PRECAUTIONARY MEASURE/S:

1. Perform hand hygiene.

STEP-BYSTEP PROCESS RESPONSIBLE PERSON/S

1. Verify physician’s order Nurse


2. Assess if the client and the family understand the reason for transfer as

explained by the physician
3. Prepare and issue duplicate copies of transfer forms. With transfer form
signed by relative/watcher, instruct to secure approval and signature of
concerned areas (i.e., Admitting and receiving ward) Nurse

Note: Clients from Delivery Room and EINC do not require Transfer slip
4. Make initial endorsement via telephone to receiving nurse about the needs

and other vital information regarding the client’s condition
5. Determine the method of transport (i.e., per wheelchair or stretcher), and the

need, for other resources for safe transport
6. Assist in transferring the client to a stretcher or wheelchair using proper body
mechanics.
Note: Mother and baby from PACU, DR and EINC are transported together in a
Nurse, Nursing Attendant,
stretcher
Midwife, Transporter
*In case of delay/deference of transfer the nurse shall inform the Admitting
Section
7. Endorse the client to the receiving nurse with complete records/documents. Nurse, Midwife
