The applicant must register first to the login page to be able to access the system

Clicking the “Please click here to download the SRF” will prompt a printable SRF auto-filled with your
registration information.
Home: Returns the user to home screen that shows the Republic Act 9442 otherwise known as
“Magna Carta for Disabled Persons and for other purposes”
ID Application: Allows you to check if the applicant has been issued an ID already. If not, click
the “Walk-in Application” button to encode.

Registry: Displays list of all active and inactive PWD IDs within your municipality
Reports: Allows you to generate different reports through the data of PWDs

User Accounts: Allows you to view and edit the information of the current user account
Downloads: A dropdown menu with downloadable forms:
● User Manual – User’s instruction guide to the system
● PRPWD Form – Printable PRPWD form for PWD walk-in applicants
● Batch Upload - By using the Batch Uploads module, you can upload or
import data in CSV files into the system in batches
● *Incident Report – In case a user encounters an error in the system
● *Service Request Form – If you want to request for training or if there’s changes
needed to be done on the user account

Batch Upload: By using the Batch Uploads module, you can upload or import data in CSV
files into the system in batches.

Change Password: Allows the current user to change his/her password (To create a strong
password it is recommended to see the “Creating Password Guide” below the page
Logout: To end the login session of the current user. Users will automatically logout after five
(5) minutes of no activity.
