Affidavit of Consent I, CRISTY NON DALDE Filipino, of legal age, single and a resident of Manito Albay, Philippines after having duly sworn to in accordance with law, hereby depose and say;
1. That I am the biological mother of KATE COOLLEENE DALDE
BASALLOTE , born on September 22 , 2005, Ten years old (10) and a minor;
2. That Kate Coolleene Dalde Basallote will be travelling to Hong Kong from September 1, 2016 to September 6, 2016 for a short vacation;
3. That she will be under the direct care and supervision of her Uncle and Auntie, Mr. WILLIAM MYRON DIGNAN and Mrs. MELODY BASALLOTE DIGNAN who will be responsible and is financially capable to provide for her sustenance and other needs;
4. That I am giving my consent and permission to my daughter, KATE
COOLLEENE DALDE BASALLOTE to travel abroad in Hong Kong together with William Myron Dignan and Melody Basallote Dignan;
5. That I am executing this affidavit to attest to the foregoing facts and for whatever legal purpose and intent this may serve.
