Dear Nurse,

Re: Mr Dallas Walters,aged 51

I am writing to refer to Walters who requires continued care for his ambulatory peritoneal
dialysis from your facility.He is being discharged today.

Mr Walters was admitted on 16 th June 2011 with the diagnosis for insertion of continuous
ambulatory peritoneal dialysis catheter .He has been chronic renal failure from last 4 years
along with Type 2 diabetes which is controlled by oral medications.During his hospitalisation
the procedure was successful and the procedure was uneventful.

Mr Walters lives with his wife,who is primary carer.He is mentally stable with mild chronic
delusions.His wife is giving supportive care.He has been totally quit smoking and drinking
since 4 years.His admission was prolonged because of his wife slow to learn the procedures
for continuous peritoneal dialysis.

In view of the above, Mr Walters needs monitoring his catheter site for infection and
educating his wife sign of peritonitis along with the importance of renal diet .It would be
greatly appreciated if you could visit his family.
