Respected Sir/ Madam,

Greetings from Krsnaa Diagnostics!
We are pleased to present this Memorandum of Understanding (MoU) between Krsnaa

Diagnostics Limited (hereinafter referred to as “KDL”) and Hospital name_name
___________________ for conducting Radiology Diagnostic tests of patients OPD and IPD.
The Memorandum of Understanding states as under:
1. Purpose:
The purpose of this MoU is to establish a collaborative partnership between the
Hospital and KDL to ensure efficient and accurate radiology diagnostic testing for the
patients referred by the Hospital.
The purpose of this MoU is to establish a collaborative relationship between KDL and
_____________ to ensure the health and safety of patients OPD or IPD through
regular and efficient diagnostic testing services.
2. Scope of Services:
i. Our collaboration will enable Hospital to offer comprehensive CT and MRI
imaging services to the patients enabling improving patient care and
satisfaction.
ii. Our diagnostics company is equipped with state-of-the-art CT and MRI
machines, that will enable the patients of the Hospital to ensure accurate and
reliable imaging results for precise diagnoses and its Treatment.
3. Responsibilities of KDL: KDL agrees to fulfil the following responsibilities:
i. Conduct Testing: KDL will conduct the agreed-upon Radiology diagnostic
tests efficiently and accurately, adhering to the highest quality standards.
ii. Reporting and Documentation: KDL will provide timely and accurate
reports of the test results to the patients, ensuring proper documentation and
confidentiality of the obtained data.

iii. Efficient Operational Procedures: KDL will work in collaboration with
[Hospital Name] to establish efficient operational procedures, including
appointment scheduling, patient flow management, and data exchange. These
procedures shall be designed to ensure seamless integration with the hospital's
infrastructure and minimize any disruptions to the patient care process.
iv. Technical Support: KDL will provide technical support to the patients of the
hospital for any queries or concerns related to the testing procedures and
results.
4. 4. Responsibilities of Hospital:: Hospital agrees to fulfil the following
responsibilities:
i. Referral Process: The Hospital will refer its OPD and IPD patients requiring
radiology diagnostic tests to KDL as per the agreed-upon protocol.
ii. Patient Information: The Hospital will provide KDL with relevant patient
information, including clinical history, medical records, and any specific
instructions or preferences, to facilitate accurate diagnostic testing.
iii. Compliance with Testing Guidelines: Hospital will ensure that all patients
either OPD and IPD shall follow the testing guidelines provided by KDL,
including scheduling appointments and providing necessary information.
5. Payment of Services:
All payments forof the services providedmade shall be made by the patients only.
There shall be no credit arrangement.
6. Limitation of Liability
KDL shall be liable solely for the veracity of the results generated vide Diagnostic
Services (“Results”) and shall not be held liable for any liability arising out of reliance
on reports and clinical information provided by Hospital and/or Patient, though the
Results may corroborate the findings of such reports and/or clinical information.

6. 7. Confidentiality and Data Protection:
Both parties acknowledge the confidential nature of the information shared during the
course of this collaboration. They agree to handle all data and information in
compliance with applicable data protection and privacy laws.
8.7. Term and Termination:
This MoU shall be effective from the date of signing and shall remain in force for a
period of 3 years. Either party may terminate this MoU by providing written notice of
2 months in advance.
8. 9. Governing Law and Dispute Resolution:

This MoU shall be governed by and construed in accordance with the laws of India.
Any disputes of the patients of the hospital, arising out of or in connection with this
MoU shall be resolved amicably through mutual discussions and negotiations between
the parties.
We will establish efficient operational procedures, including appointment scheduling, patient
flow, and data exchange, ensuring seamless integration with the hospital's infrastructure.
Please signify your agreement to the terms of this MoU by signing and returning a copy of
this document.
