1 Introduction
MMC-Gamuda KVMRT (T) Sdn. Bhd. (MGKT) has been appointed by MRT Corporation (MRTC)
as the Works Package Contractor for the KVMRT Underground Works SSP Line 2 UGW under
a design and build contract. Their Lead Design Consultants (LDC) is Aecom Perunding Sdn.
Bhd. and the Detailed Design Consultants (DDC) for the respective Shafts, Tunnels and South
Portal is as follows:

● TU1 to TU8
Effective from May 2019 under the new Turnkey Contract (TC), the supervision teams of the
DDC’s (formerly known as the Supervision-In-Chief) are integrated with MGKT IQC personnel
to carry out construction supervision activities for the underground works.
The purpose of this report is to provide an overview of the activities and progress by the TC
Supervision (TCS) Teams during October 2019. Note that all the information in this report is
updated for October 2019, up to 31st October 2019.
2 DDC 3 Shaft & Portal Package – ESC1, ESC2, ESC3, IVS2, IVS3 & DEWP
2.1 Major Issues
No major issue on shafts for the month of December 2019.
2.2 Organisation
TCS staffing for DDC3 was announced with issuance of MGKT Quality Notice ref. no. MGKT-
QHSE-QN-005 dated 25th April 2019 with attached TC Supervision (TCS) Organisation Chart. At
the end of January 2020, the SC organisation stood at 16 people for this section of work. 0
staff joined the project this month:-

● Construction Coordination meetings at each site have been carried out with MGKT operation

require design assessment and clarification of details. Wherever possible issues are being
solved at the site level or via discussion and emails.

Operations, I&M team and others.
● Coordination and interfaces are carried out in various meetings as shown in item 2.7.6.
2.5 Technical
2.5.1 ESC1 Technical / Technical Queries (TQ)
No new TQ this month & no outstanding TQs.
2.5.7 SSP-MGKT-TECHQ-002400 on temporary opening in roof slab for TBM retrieval. Responded
by DDC3 on 9 January 2019.
For details of each TQ, refer to the TQ Register managed by MGKT D&T and included in
Appendix 2.
For I&M details refer to weekly Interpretive Monitoring Reports (IMR) submissions which are
reviewed every Wednesday in the weekly Tunnel/Shaft IMR meeting with MGKT & MRTC.
o DEWP
Strain Gauge at Zone 1 of South Portal breached Alert level, but it’s fluctuating and suspected due
to the ongoing excavation & strut installation works. And the adjacent inclinometer and
ground settlement markers show stable reading. Further to the site inspection no
anomalies is observed surrounding the area.

2.7 QA/QC
As TCS now effective since May 2019, new document submissions such ITP and inspection
forms shall adopt the new inspection format. RFIs shall be revised to combine IQC & DDC
under SC. Concurrently until the new formats are available, existing inspection points for IQC
& LDC/DDC shall be attended by the combined SC organisation.
ESC3 has adopted a precast panel construction method and panels are produced at the GIBS
manufacturing facility near Sepang. The inspection at the manufacturing factory is carried
out by MGKT QA team. The check at the delivery and installation stage will be done by TCS
QC team. The check sheet is being developed now for the 1st delivery and installation
commenced in November, 2019 and is on-going.
It was noted that the workmanship of skin wall construction at ESC1 and ESC2 is poor, with
voids and honeycombing, and requires further improvement. Defects such as honeycomb,
exposed rebar, water leakage, crack lines and insufficient concrete level are observed. NCRs
have been issued due to these defects. Approved defects rectification works are currently
on-going.
The quality of workmanship for the construction of interface structure at IVS3 is poor,
resultng in water leakages and honeycombs. NCR has been issued with rectification works in
progress.
2.7.1 Reviews

SSP-MGKT-TECHQ-002400 at DEWP – BMSL – Temp. opening in roof slab for TBM retrieval
responded by DDC3 on 9/1/2020.
SSP-MGKT-TECHQ-002426 for all TU line wide - Omission of crack inducers for first stage
concrete responded by DDC3 on 13/1/2020.

Appendix 2.
On-going daily convergence monitoring at interface between TU3-KBNS and once a week
joint inspection with TU3 MGKT Operation team & Subcontractor and surveyor/I&M at
effected segment lining.
Some sinkhole or heaving occurred in January, 2020 are recorded in IMR as follows: -
o TU5
Various numbers of AAA breach on existing GSM, BSM and cracks meter at Kompleks Budaya
Kraf during TBM passed through. New crack meters and BSM proposed by DDC to monitor
cracks and ground settlement. Propping system was proposed on nearby column CTR-BSM11-
006 to minimize cracks occurring on existing beam.
On 26/1/2020, sinkhole size of approximately 2.0m x 2.0m x 3.0m occur in front of Kompleks
Kraf. Sinkhole area have been cordon off & immediate backfill by ERT team.
Repairing and reinstatement of all cracks and defects is ongoing by MGKT Operation team.
Building inspection where accessible was carried out in weekly basis among TCS, I&M and
MGKT operation to inspect and record new impact/defect to the building during TBM pass
through.
o TU1
On 21/12/2019 at 3.25am, a sinkhole (approx. size of 7.0m x 5.0m x 1.5m) occurred at Jalan
Sultan Azlan Shah (middle lane). Affected area was immediately cordoned off and traffic team
was informed. TU1 site team immediately mobilize in to backfill the area for rectification
work.
On 23/12/2019 at about 04.00am, some heaving at Jalan Sultan Azlan Shah .(middle lane)
occurred. Affected area was immediately cordoned off and traffic team was informed.
There is a 350mm crack length in the affected area. No slurry bubble or leak observed at the
time of inspection. All inspection recorded in SVL report SSP-MGKT-UNGW-TU1-SVL-00048.
On 24/12/2019 at 11.30am, some depression and lead to sinkhole occurred at Jalan Ipoh road
under TBM S-1056 alignment. Affected area was immediately cordoned off and traffic team
was informed. About 200 cubic meter of sand mixed with crusher run was backfilled.
On 1/1/2020 a (2nd.)sinkhole size of 2.5m X 5.0m X 1.5m occur at (CH 25450) Jalan Sultan
Azlan Shah with temporary road diversion and control by TMO.
On 2/1/2020 a (4th) Sinkhole size of 6m x 5m x 3m is found on the fast lane of Jalan Ipoh (CH
25396) & the previous sinkhole (3rd sinkhole - CH25450) was collapsed at the same time.
All affected sinkholes area was immediately cordoned off and backfilled.
Grouting work was carried out by MGKT grouting team with 32nos of compaction grout and
49 nos and 47 nos of rock fissure grout applied within sinkhole effected area for SB and NB
respectively.
The strain gauge for CP4 breached the alert level showing fluctuation reading. The cause of
movement was investigated and no crack observed around D-Frame especially on the
segment lining. No joint leak on segment lining and no excessive water ingress at the rock
face during the site inspection. The cause of movement is likely due to the Cross passage
construction activities such as rock splitting for rock excavation and mechanical breaking
works
There was a convergence OP breached alert and alarm level at CP4 NB and CP1 SB. However
nearby step monitoring still no movement recorded and no physical significant effected in
segment was observed.

3.7 QA/QC
As TCS now effective since May 2019, new document submissions such ITP and inspection
forms shall adopt the new inspection format. Inspection methodology still remains as before
TCS started. Concurrently until the new formats are available, existing inspection points for
DDC shall be attended by RE/ARE while inspection record under QC Engineers shall be
referred to through the Field View paperless platform organized by MGKT QA Dept.
LDC SC SRE conducted the meeting for the tunnel outstanding NCR for TU4 & TU5 on 10 th
January, 2020 with ARE and QC engineer in order to expedite the NCR closing procedure.
