1996 as amended by Proclamation No. 172 and Memorandum Order No. 119, both dated October 16, 1987 in
relation to the pertinent provisions of Commonwealth Act No. 141, as amended (Public Land Act);

Order No. 119;
8. I understand that any applicant who has willfully and knowingly submitted false statement or executes a false
affidavit in connection with this application shall be deemed guilty of perjury and be punished accordingly. I shall
also be held liable by fine or imprisonment or both, the amount and duration of which shall be determined by a
competent authority, if I induce or permit another person to file the application form in behalf of an unqualified
applicant. Any amount paid on account thereof shall be forfeited in favor of the government and I will no longer
be entitled to apply for any private or public land in the Philippines.
