II. I am Paulino P. Tado, currently conducting a study entitled, “Influence of


Self-learning Module Dimensions and Parental Involvement on the Learning
Outcome of Students in Mathematics”. The data gathered from this
questionnaire are of great help to finish and fulfill this academic endeavor.
