1. That we are the biological parents of RYAN VIC
SECRETARIA ANICETO who was born on 5 February
2007 at Santa Maria, Bulacan.
2. That ANNALIZA VALEROSO ANICETO is the sibling
of RYAN VALEROSO ANICETO and auntie of RYAN
VIC SECRETARIA ANICETO.
3. That we are giving our full consent for our minor child to
travel from the Philippines to other countries and
territories along with his auntie ANNALIZA VALEROSO
ANICETO.
4. That we are giving full consent to ANNALIZA
VALEROSO ANICETO to procure tickets for the travel/s
of our child.
5. That we are attesting that ANNALIZA VALEROSO
ANICETO is willing and able to maintain and support my
child financially and guarantee that he will not become a
public charge or burden during his travel to other
countries and territories.

ATTORNEY-IN-FACT, ANALIZA VALEROSO ANICETO, single, of legal age,
with residence and postal address at Block 69 Lot 1 Phase 2, Woodlands of Grand
Royale, Barangay Bulihan, Malolos City for me and in my name, place and in my behalf,
and to do and perform all of the following responsibilities and to have all rights in
connection with the following:
1 . To assume responsibility for the issuance/application for the renewal
of the passport of our minor child JAMES BRYAN ANICETO and for
allowing the trip of the said minor/s and further assume all obligations
consequent thereto.
2. To assume responsibility for the issuance/application of the renewal for
APOSTILLE purposes.
3. To perform and act as in and for me in a parental capacity to our child
JAMES BRYAN ANICETO including but not limited to accompanying
him in his trip from the Republic of the Philippines to other countries and
territories, to procure tickets for his trip, and take full responsibility for
his expenses, subsistence, and accommodation throughout the duration of
his stay in the above mentioned country.
4. Give consent and permission for any kind of medical care and
treatment, and to sign any papers that are needed to have the above
mentioned child admitted to a hospital for such purpose, or as may be
required to maintain the health of JAMES BRYAN ANICETO, our
biological son.
5. Give consent and permission for enrollment in and admission to school
and to resolve any problems that may arise from school attendance, and to
sign any papers that are necessary for such purpose or sign other
documents relating to the child’s welfare at school including any and all
educational decisions that need to be made for JAMES BRYAN
ANICETO, whom I have legal custody of and perform any act that is
necessary to obtain relief or aid that may benefit our biological son
JAMES BRYAN ANICETO.
6. Perform any other acts for support, health, and general care of my child
JAMES BRYAN ANICETO as may be required or necessary.
7. It is not at all my intention to relinquish our full parental rights to
JAMES BRYAN ANICETO.
HEREBY granting and confirming all of the foregoing matters which our
attorneys-in-fact may do or cause to be done in the premises, with the same force and
effect as if we were the one doing the same personally.
