    MRI OF CERVICAL SPINE

    Brief Clinical Notes: Known case cervical myelopathy & HTN. H/O neck pain radiating to right upper limbs.

    Techniques: Multiplanar & multisequence MRI of cervical spine was performed.

    Findings: (Reporting on films and PACS).

    Loss of cervical lordotic curvature is noted.

    The vertebral alignment is within normal limits.

    Marginal osteophytes are noted in C2-C7 vertebrae.

    Rest of the vertebral bodies appear normal in height and demonstrate a normal pattern of marrow signal
    intensity.

    Focal signal intensity change is seen in cervical spinal cord at C4 vertebral level; appear hyperintense on
    T2W image suggesting myelopathy.

    Endplate signal intensity change noted in C6/C7 vertebrae; appear hypointense on T1WI, hyperintense on
    T2W / STIR images – suggesting Modic type - I change.

    C2-C3: Normal signal intensity in the disc with normal disc height is seen. Diffuse disc bulge with post erior
    osteophyte indenting ventral subarachnoid space. Intervertebral foramina are normal on both sides.

    C3-C4: Desiccated disc with reduction of disc height is seen. Central and both paracentral annular disc bulge
    with posterior osteophyte indenting ventral subarachnoid space. Intervertebral foramen is narrowed on right
    side. Mild spinal canal stenosis is noted.

    C4-C5: Desiccated disc with normal disc height is seen. Diffuse disc bulge indenting ventral subarachnoid
    space. Intervertebral foramina are normal on both sides.

    C5-C6: Desiccated disc with normal disc height is seen. Central and both paracentral annular disc bulge wit h
    posterior osteophyte indenting ventral subarachnoid space. Intervertebral foramen is narrowed on right side.

    C6-C7: Desiccated disc with reduction of disc height is seen. Central and both paracentral annular disc bulge
    with posterior osteophyte indenting ventral subarachnoid space. Intervertebral foramen is narrowed on right
    side.

    Whole spine screening:


    Loss of lumbar lordotic curvature is noted.
    Marginal osteophytes are noted in L2- L5 vertebrae.
    Disc desiccation is seen at variable levels.
    Mild disc bulge is noted at L3-L4, L4-L5 & L5-S1 levels indenting ventral thecal sac.
    Lumbar & dorsal spinal cord is normal in morphology and signal intensity.

    Impression:
    1. Suggestive of cervical myelopathy at C4 vertebral level.
    2. Cervical spondylosis with:
    At disc levels:
    • C3-C4: Desiccated disc with reduction of disc height. Central and both paracentral annular disc
    bulge with posterior osteophyte indenting ventral subarachnoid space. Intervertebral foramen
    narrowed on right side. Mild spinal canal stenosis.
