IV. NUMBER OF VEHICLE INVOLVE: Two (2) Road Condition /
Intersection
1. On December 31, 2021 at about 8:00 AM, a Vehicular accident
transpired along the National road Brgy. Angustia, Nabua, Camarines Sur involving
one Honda motorcycle with Plate No.1301- 02 driven by Jaymark Vasquez y
Panganiban, 33 years old, married, resident of San Roque, Bato, Camarines Sur and
one Honda TMX trimobile with plate No:050101, driven by Jay Caňeso y Talagtag, 21
years old, resident of Tagpolo Bato, Camarines Sur.
2. Investigation conducted disclosed that on or about 8:00 AM of December
31, 202, a vehicular accident transpired along the national road, Angustia, Nabua,
Camarines Sur where in a honda ACB125FI motorcycle, Color Red/Black, bearing
Plate No: 1301- 02, while travelling route to their residence from Nabua Camarines
Sur upon reaching national road of said place of incident and while executing left
turned, Accidentally bumped by the Honda TMX TRIMOBILE, color black bearing
Plate No: 050101 while travelling route to Sto. Domingo, Nabua Camarines Sur from
their residence lost control his driven vehicle rammed to a rice field owned by
unidentified person.

Intersection
Panganiban, 33 years old, married, resident of San Roque, Bato, Camarines Sur
and one Honda TMX trimobile with plate No:050101, driven by Jay Caňeso y
Talagtag, 21 years old, resident of Tagpolo Bato, Camarines Sur.
Camarines Sur where in a Honda ACB125FI motorcycle, Color Red/Black, bearing
Plate No: 1301- 02, while travelling route to their residence from Nabua Camarines Sur
upon reaching national road of said place of incident and while executing left turned,
Accidentally bumped by the honda TMX TRIMOBILE, color black bearing Plate No:
050101 while
3. Action Taken: PINSP EARL KRISTIAN T. LAYONES and SPO2 RUBEN
MIKE N. COLICO, investigator respondent and conducted the investigation and
procedure at the scene of incident after the driver/victim was on their way to their
home from Nabua buying some groceries and stuff for new year eve while the other
one is going to Sto. Domingo to do the same thing but the accident occurred.
4. The one (1) victim/driver Jay Caňeso y Talagtag are currently detained at
Nabua Municipal Police station in a case of reckless driving in violation of Republic Act
4136 of the revised Penal Code is pending for trial.
5. Final Report will follow.
