GUIDELINES FOR PREPARING SAFETY PERFORMANCE REPORT


MONTHLY SAFETY PERFOMANCE REPORT
Month June- 2024.

This format is generated for standardizing the information and following


guidelines should be used.

1. Accident recording of four mentioned categories should be done. In


current month accident information of recorded categories as the case
may be sent, but following months accident recording should be done
for all mentioned categories.
2. Safety performance can be sent in one of the two suggested ways.
3. Details of other training programs may be given by adding new rows.
4. Safety meetings are required to be held as per the statutory norms and
therefore its records shall be maintained properly.
5. Safety violations notice during safety inspections should be recorded
and corrective or preventive action shall be taken.
6. All works in which risk is involved should be carried out under work
permits and their records should be maintained.
7. Records of internal and external audits should be documented.
8. Records of client’s complaints should be documented.
9. Where records of any item mentioned in the new format is not available
in the current month it can be shown “Nil”, however efforts should be
taken to record information in following months.
10. Details of safety function, motivational schemes etc. can be given in
Octarate annexure.
11. All supporting data must be readily available at site, from which
performance report is prepared.
12. Refer attachments for the basis of various indicators.
