AFFIDAVIT OF HEIRSHIP with SPECIAL POWER OF ATTORNEY
We, DIONISIA P. NOTARTE, JULIE N. NOTARTE, LUCY N. CUSTODIO,

ARNEL A. NOTARTE, BENEDICTO A. NOTARTE, JOSIE N. SAVIOR, LITO A.
NOTARTE, MICHELLE A. NOTARTE, and REY A. NOTARTE, all of legal age, Filipinos
and residents of, Purok 7-A Pag-asa Kapalong Davao del Norte, do hereby depose and
manifest THAT:
1. We are the surviving heirs of the late BENEDICTO F. NOTARTE who died on
DECEMBER 06, 2019 in DRMC TAGUM CITY, with DIONISIA P. NOTARTE being his
wife while JULIE N. NOTARTE, LUCY N. CUSTODIO, ARNEL A. NOTARTE,
BENEDICTO A. NOTARTE, JOSIE N. SAVIOR, LITO A. NOTARTE, MICHELLE A.
NOTARTE, and REY A. NOTARTE, as her sons and daughters;
2. Our legal relation to the deceased can be evidenced by competent public official
documents such as marriage and birth certificates;
3. The Title of the agricultural land of the family was awarded and granted through the
HANDOG TITULO Program that was held on September 11, 2024 at Kapalong Town
Square;
4. By reason of his death, he shall be substituted by herein heirs' authorized
representative duly constituted by these presents;
5. Due to immobility, age and separate residences, I DIONISIA P. NOTARTE, hereby
and by these presents, name, constitute, and appoint our co-heir DIONIA P. NOTARTE,
to be our true, lawful and sufficient attorney-in-fact for us and in our name, place and
stead, to do and perform the following and things:
a. To transact with proper governmental agencies and offices in relation to the said
property particularly to secure the Owner’s copy of the Title that was awarded and
granted through the HANDOG TITULO Program held on September 11, 2024 at
Kapalong Town Square;
b. To request, execute, claim, and sign any document in connection with the said
property;
c. To perform such other acts or things incidental to the above powers granted to render
the same fully effected.
