Hernia

Definition
Abnormal protrusions of a viscous through a normal or abnormal opening in a cavity.

Parts of Hernia:-

 Cover.
 Sac.
 Contain.

Types of hernia

 Inguinal hernia.
 Umbilical hernia.
 Epigastric hernia.
 Incisional hernia.
 Femoral hernia.

Inguinal hernia divided to direct, indirect.

Anatomy of inguinal canal


Inguinal ring, inguinal fascia, external oblique, aponeurosis, conjoined tendon, transvers fascia.

Equipment
1) Instrument; General set.
2) Suture;
 prolen NO 2/0 or 3/0. CBN for skin.
 Prolen NO 1 RBN for repair.
 Prolen NO 2/0 RBN for mesh fixation.
 Vicryl NO 0 FOR Sac ligation.
 Vicryl NO 2/0.
3) Surgical blade NO 15, 20.
4) Diathermy monopolar.
5) Sterile gloves.
6) Gauze small 4*4.
7) Anti-septic iodine.
8) Drapes.
9) Sterile mesh.

Instrument on mayo table.


Scalpel handle No 4. Forceps tooth 2 pcs.
Sponge holder . Forceps non tooth 2 pcs.
Artery forceps mosquito 4pcs . Babkook 2 pcs.
Artery forceps medium 2pcs . VaraPof retractor 2 pcs.
Scissor tissue . Galipot.
Scissor stitch . Needle holder 2pcs .

Anesthesia : General, spinal, or local filtration.

POSITION: Supine.

Procedure
Inguinal hernia repairs are of the following three general types;

 Herniotomy; remove of hernia sac only.


 Herniorrhaphy; remove of hernia + repair of posterior of inguinal canal.
 Hernioplasty; remove of hernia sac + enforcement artificial mesh.
 After skin incision opening of the subcutaneous fat along the line of incision.
 Opening of the deep fascia.
 Division of the external oblique aponeurosis and expose inguinal canal.
 Mobilization of the spermatic cord.
 Identification and isolation of the hernia sac.
 Placement and fixation of mesh.
 Hemostasis.
 Closure of spermatic cord layers, the external oblique aponeurosis, subcutaneous tissue,
and the skin.

Other approaches to open inguinal hernia repair include the following;


 Bassini repair.
 Shouldice repair.
 Plug- and- patch repair.
 Prolene hernia system (PHS).
 Mcvay repair.
