NURSING SHIFT HANDOVER

• Patient handover is a technique or a way to


convey and receive something (report) related
to the patient's condition.
General purpose
• Communicate the patient's condition and convey important
information.

Special purpose
• 1. Convey the patient's condition and state (focus data).
• 2. Delivering things that have / haven't been done in nursing
care to patients.
• 3. Convey important things that must be followed up by the
next service nurse.
• 4. Prepare a work plan for the next service.

For Nurses
• 1. Improve communication skills between nurses.
• 2. Establish a cooperative and responsible
relationship between nurses.
• 3. Continuing implementation of nursing care for
patients.
• 4. Nurses can follow the development of patients
in a complete manner
For patients
• Patients can raise problems directly if something
has not been revealed.

PREPARATION
• 1. The handover is carried out every change of
shift
• 2. The principle of handover is that all patients
who have just entered, and patients who
handover, especially patients who have problems
that have not / can be resolved, and who need
further observation.
• 3. The associate nurse delivers the handover to
the executive nurse (who receives the delegation).
THINGS THAT NEED TO BE CONVEYED IN THE HANDOVER
ARE :
• a. general aspects
• b. number of patients;
• c. patient identity and medical diagnosis;
• d. data (complaints / subjective and objective);
• e. nursing problems that still arise;
• f. nursing interventions that have and have not been
implemented
• g. collaborative and dependent intervention;
• h. general plans and preparations that need to be done, for
example: (preparation of operations, supporting
examinations, and other programs)
IMPLEMENTATION
At the nurse station
1. Both service groups are ready.
2. The group that will be in charge of preparing the
notebooks.
3. The head of the room or the team leader opens the
handover ceremony.
4. The information submitted by the nurse must be
clear, concise and systematic
5. Nurses in service can clarify, ask questions and
validate things that have been handed over, and have
the right to ask about things that are unclear.
 
In the patient bed
• 6. The head of the room conveys greetings, then
the implementation nurse asks the patient's
basic needs.
• 7. Nurses who are on duty subsequently, fully
review the nursing problems, needs, and actions
that have / have not been implemented, as well
as other important matters during the treatment
period.
• 8. The things that are special, and require careful
details should be noted specifically, and then
handed over to the next nurse.
Post Handover
At the nurse station
• 1. Discussion.
• 2. Reports for handover are written directly in
the handover format, which is signed by the
current Nurse, and the Nurse to be the next
service, and is known by the Head of the
ward.
• 3. The handover process is closed by the head
of the ward or the team leader.
