Community
Milton City, of Santa Rosa County in Florida was the community of interest in
the assessment. I narrowed it down the cheval community located at 5870
Cheval Street.
It is a rural community with a population of 12,839.
The largest ethnic group or race is White at 53.6%, followed by 23.6%
Hispanics. The Asian race is the least at 10.7% (Florida Cities by Population,
n.d.).
The people in the community have access to schools, religious centers,
recreational facilities, parks, veterinary services, markets, and restaurants. It is
a quiet community full of hospitable people of all ages with a median age of
39.9 years.
It is in the neighborhood of Arcadia Street, Gainer Ave, and Morningside Ln.
Community Assessment Model

This model helps the community in

identifying health needs within and
addressing them with resources that
are available (Stanhope et al., 2020).
The model allows the whole
community to participate in the
program, thus building confidence and

the community.
TRENDS
Substance abuse is a huge behavioral health problem in Milton city,
with a report of 100 deaths from drug overdose in 2020.
In comparison to the entire state of Florida, there was an increase
of 150% of opioid-related deaths in 2020 from 2013. The number
of hospitalizations due to opioid abuse also increased that year.
Nationally, substance abuse is recognized as a problem. Across the
states, opioid abuse and addiction are the major health concerns.
The mortality rate in the US increased by 30% from 2019 to 2020.
Approximately 19.4 million adults were diagnosed with substance
abuse disorder as reported by the National Survey on Drug Use and
Health
Substance abuse is a major health issue affection populations at all

resources that provided common concern among the 3 was substance abuse: inaccessibility of
rehabilitation services to the unhealthy behavior of drug and substance abuse treatment due to
residents struggling with substance abuse among teens and scarce resources within the

the county. and understanding of substance abuse as
a health issue rather than a moral failing.
Contacted different agencies for
partnership and resource support.

Action!
Provided the leaflets to community members by handing them out to community social settings like
eatery places, and local convenience stores. They are easily acquired and portable as they can be
carried in pockets, bags, purses to allow them to read and reference them later. Put up posters in
social settings and high traffic areas because they are visible and anyone can read them.
This phase allows for an assessment of
the effectiveness of the strategies and

Evaluation Phase
provides an opportunity to make
necessary adjustments to improve their
impact.
The evaluation phase for this project
would entail further investigation into
the needs the community regarding
substance abuse.
Another survey will be carried four
months down the line to measure
changes in attitudes of people towards
individuals struggling with substance
abuse disorder, changes in
stigmatization, and the level of
implementation of campaign and
education programs.
The substance abuse treatment centers and
partner agencies will take part in justifying
the need for increasing availability of
resources by providing data on those who
have utilized their services from the
studied zip code.
Establish if the community got funding
and support opportunities for development
of new treatment facilities.
The windshield survey and community interviews
were tasks I did well.
I addressed the issue of awareness,
stigmatization, and finding accessible facilities
for treatment and recovery of substance and
alcohol abuse disorder.
Given another opportunity I will do an
investigation on the social determinants of health
that cause substance abuse, and develop harm
reduction strategies like overdose prevention
programs to decrease the detrimental effects of
substance abuse.
