‘NIKSHAY”
• To keep a track of the TB patients across the country, the Government of India has
launched a system called NIKSHAY.
• NIKSHAY covers various aspects of controlling TB using technological innovations.
Apart from web-based technology, SMS services have been used effectively for
communication with patients and monitoring the programme on day-to-day basis.
• . ICT enabled adherence mechanisms [99DOTS, MERM, refill monitoring, VOT
(manual)] will feed treatment related information from patients and treatment
supporter into Nikshay enriching it as a patient care and support platform to prioritize
patients for differential care.

Directly Observed Treatment, Short-course (DOTS)


• The DOTS strategy along with the other components of the Stop TB strategy,
implemented under the National Tuberculosis Elimination Programme (NTEP) in
India, is a comprehensive package for TB Elimination.
• The DOTS strategy is cost-effective and is today the international standard for TB
Elimination programmes. To date, more than 180 countries are implementing the
DOTS strategy. India has adapted and tested the DOTS strategy in various parts of
the country since 1993, with excellent results, and by March 2006 nationwide DOTS
coverage has been achieved.
Medication event reminder monitor (MERM) for
DR-TB patients.
 The MERM is a digital pillbox that has been designed to monitor MDR-TB
treatment in resource-constrained settings.
 Medications are dispensed in blister packs and each drug is placed in a
different partitioned compartment within the pillbox.
 The MERM is programmed to provide audio-visual reminders to the
patients to take medications at a specific time of the day.
 The visual reminder consists of:
 Green light: Corresponds to a label encouraging the patient to take a dose.
 Yellow light: Alerts patients about the need to refill medications.
 Red light: Indicates a low battery and the need to replace the MERM’s
battery.
 The audio reminder consists of a ringing sound that would occur at the
same time as the visual dose-taking reminder.
 The data from the MERM boxes is transmitted every 72 hours using
cellular networks and recorded on a computer server..
