Through these mechanisms, traditional Chinese medicine regulates the expression of genes and proteins to inhibit the inflammatory response and oxidative stress, promote the proliferation of gastric mucosal epithelial cells, and even prevent cancer development. Traditional Chinese medicine reported in this review has inhibitory and bactericidal actions against H. pylori and increased efficacy in improving histological lesions and the clinical picture. The adoption of these traditional therapies, used by millions of people in different countries over several centuries, should finally receive due attention from the scientific community, pharmaceutical companies, and health authorities of developed and developing countries alike. The cost of treating Helicobacter pylori infection and dozens of other digestive diseases, including inflammatory bowel disease, is becoming increasingly prohibitive. These treatments offer an alternative that is effective and affordable. In the future, any side effects that may arise mainly from long-term administration will need to be studied in greater detail. After being adequately trained, doctors should be aware of the toxicity of traditional Chinese medicine during such clinical trials. Traditional Chinese medicine is expected to increase as long as research at the molecular, genetic, and pathophysiological levels continues and the complex mechanisms of action of the substances contained in traditional medicine preparations are clarified. As the authors rightly note, parasitic infections are nowadays a significant public health risk with a high economic burden. The epidemiology of parasitic infections varies in different geographical regions of the world. The incidence of parasitic infections is increasing due to the possibility of easy communication of people through travel to countries where parasitic infections are endemic. As regards Europe, existing data suggest an increase in all epidemiological parameters related to parasitic infections in general and liver infections in particular. The article suggests and underlines the need to organize public health programs to raise awareness among society and health professionals about good hygiene and food handling practices. Avoiding uncooked meat and seafood, careful hand washing, swimming in lakes, and drinking water from cisterns or wells could significantly reduce the economic impact by preventing hospital admissions and rationalizing medical diagnostic and invasive treatment procedures. I fully agree with the report that international organizations should pay particular attention to this issue. Furthermore, infectious disease specialists and gastroenterologists should be more actively involved in these infections' various diagnostic and therapeutic aspects. The idea is interesting , but there are some observations to make. First of all the author forgot a simple, not invasive, physiologic and reliable method to measure gastric emptying, that is real-time ultrasonography, which allows the use of a normal solid–liquid meal and gives an immediate result (1). Furthermore the author did not take into account the fact that, as indicated in figure 1 of ref. 4, the cause of delayed gastric emptying in gastroparesis may be also due, not only to antral dysmotility detectable with BSGM, but also to impaired pyloric relaxation upon arrival of antral peristalsis (pylorospasm) and, sometimes, to dysmotility of duodenum, which does not accept the bolus coming from the stomach. The pylorospasm, which was observed in 14 of 24 diabetics, by Mearin et al (2) forms part of the widespread disruption of gut motility that affects some patients with diabetes. Unfortunately these pyloric and duodenal motor abnormalities are not detected by the technique of BSGM, which could find a normal antral motility, giving the impression that everything is OK, while the gastric emptying is already delayed. Therefore I would suggest to monitor the diabetic patients with the real-time ultrasonography, which is able to detect also a minimal delay in gastric emptying harbinger of gastroparesis, and, if the gastric emptying is delayed, then perform the BSGM, to see if the cause of the slowing is due to antral dysmotility. If the latter examination is negative, then the cause of delayed gastric emptying shoul be pylorospasm or duodenal dysmotility.
