Immunization Requirements
Colorado law requires all students attending Colorado schools and licensed child cares to be vaccinated against certain diseases unless a medical or non-medical exemption is filed. Medications at School
- In order to administer any medication at school, prescription or over-the-counter, parents/guardians must submit a Medication Administration Authorization form for each medication to be administered. - Authorization forms are good school year to school year. Medication must come in its original, properly labeled container, and brought in by the parent or guardian. - You may download the medication form from this website under the medication tab or by clicking on the above link. High School students may possess and self administer certain medications during the school day. More information may be found under the medication tab or by contacting your school nurse. EXCEPTION: A student who requires the use of emergency rescue medications may be allowed to self-carry. Your health care provider can give permission to self-carry on the medication authorization form. More information may be found under asthma or allergy tabs.
Vision and Hearing Screening
In accordance with C.R.S. 22-1-116, the sight and hearing of all children in the kindergarten, first, second, third, fifth, seventh, and ninth grades, or children in comparable age groups referred for testing, will be tested during the school year. Parents may opt out of the screenings by submitting their requests in writing to the School Registered Nurse.
