    2. Filing Fee

    P 1,000.00 (for non-metallic mineral)


    P 2,000.00 (for metallic mineral)

    Umpiring Fee

    P 6,000.00

    3. Copy of the valid and existing Industrial or Commercial Sand and Gravel Permit
    where the aggregate materials were taken
    4. Proof of payment of Excise Tax of the aggregate materials to be exported
    5. If the EXPORTER is not a holder of sand and gravel permit, a Certificate of
    Accreditation as Trader or Dealer of Minerals / Mineral Products should be secured
    from the MGB-1. Hereunder are the requirements for the said Accreditation:
    5.1 Filing Fee: Php 5,000.00
    5.2 Duly accomplished application form (MGB Form 16-1)
    5.3 Copy of the permit / contract of the suppliers / sources of mineral / mineral
    products / by-products or a copy of Certificate of Accreditation in case the
    source of materials is a trader, dealer or retailer.
    5.4 DTI Registration
    5.5 Supply / contract agreement with a permit holder / contractor / accredited
    dealer producing the specified minerals / mineral products / by-products;
    5.6 Marketing / Sales Agreement / Purchase Order / Supply Contract / Pro-forma
    Invoice with buyer of minerals / mineral products / by-products.
