2015 4/16 Medical Records Technician
Process and maintain inpatient, outpatient and ambulatory procedure unit medical records for the 5 th
Medical Group at a dual wing/Personal Reliability Program installation. Compile and extract medical
records data to ensure compliance with regulatory requirements. Excellent skills in reviewing and
understanding medical terminology. Possess a strong understanding of medical lab requirements as it
correlates to members medical condition. Gather data for the daily pull list and preparing reports for 40
patients or more on a daily basis. Responsible for preparing and transferring medical records for 20
active duty members and their families per week. In-process 50 active duty members on a weekly basis.
Responsible for filing and securing medical information in the electronic medical record/Composite
Health Care Systems. Conduct quality assurance checks on 84 hard copy records on a monthly basis to
ensure they are accurate and in compliance with all HIPPA codes and legal guidelines. Responsible for the
upkeep, proper filing and securing of 600+ patient medical records. Military Filing System - by sponsor
social security number, terminal digit order, color-coded and blocked filing system. Contents of a military
medical record, layout, and sections family member prefix designation forms used in a military treatment
facility, and the medical record tracking procedures. Maintain medical records in accordance with
prescribed directives, and file military forms documenting patient care into patient medical records. I
conduct a daily pull; delivery and retrieval of 1,372 patient records monthly; 16,464 yearly. Prepared
reports regarding record statistics as necessary, participated in records review as part of the facilitys
quality assurance program and in accordance with accreditation standards. Proficient in retiring medical
records in accordance with Air Force guidelines. Active security clearance on file and possess CAC.
2013 - 2015 Release of Information/HIM Technician
Retrieved all patient medical information through various media resources including microfilm,
warehouse storage, satellite medical facilities, microfiche the electronic health record and billing
documents as necessary. Skilled in gathering all protected health information in accordance with specific
medical request. Responsible for the release of protected health information as requested through
electronic, CD or hard-copy distribution. Very knowledgeable in HIPPA Policy guidelines. Excellent
experience in using STAR, MISYS and Cerner medical programs. Generated invoices per requestor in
accordance with facility and North Dakota release of information billing guidelines. Processed over 2,000
medical requests including legal, insurance, continuing care and urgent requests as needed for patients
being airlifted to another medical facility. Skilled in using medical terminology to process all medical
requests. Excellent office administration, receptionist and clerical duties. Answered an average of 25
calls per day which were recorded and logged through Cisco electronic software. Possess knowledge of
correct grammar and punctuation skills to accurately edit written documents and reports. Uses appropriate
software to prepare business or government forms. Resolved and/or clarified conflicted or missing
information in patients charts by consulting with doctors as well as coding staff. Protected the security of
medical records to ensure confidentiality was maintained.
2012- 2013
Assisted the Director and Regional Director in the following: Establishing monthly meal plans for
children ages 1-5. Responsible for coordinating staff training schedules. Collected 100 Childcare
Payments weekly. Managed and supervised a staff of 13 teachers and 3 Security Guards. Responsible for
recruitment and hiring of staff. Ensured the safety of over 100 children ages 0- school age. Ensured and
established a program environment which promoted positive interaction among children, adults and staff.
Proactively suggested progress improvements for facility and children. With accuracy, established and
maintained training records and ensured all staff were current on CPR/First Aide. Responsible for
scheduling all social service training courses. Created over 100 records for children tracking: growth
development, gross and fine motor skills and behavioral development. Created monthly lesson plans for
each class. Analyzed and observed classroom daily operations, to ensure educational standards were
upheld. Submitted all reports to Regional Director.
