COMES NOW petitioner, unto this Honorable Court, most respectfully

avers:
1. That petitioner is of legal age, Filipino, single, resident of San Juan,

Sergio Osmeña, Zamboanga del Norte, and maintains a regular place of
work at General Luna Street, Dipolog City;
2. That he is a member in good standing of the Philippine Bar,

Integrated Bar of the Philippines and the Local IBP Chapter where he
belongs;
3. That he possesses all the qualifications and none of the

disqualifications for the Office of a Notary Public;
4. That he was never convicted of any crime involving moral turpitude;
5. That the following are the pertinent data in connection with this
petition;

executive officers of the IBP, Local Chapter, copies of which are hereto
attached to form part of this petition;
7. That attached hereto are clearances from the Office of the Bar
Confidant of the Supreme Court and the Integrated bar of the Philippines
to form part of this petition;
8. That attached hereto is the receipt of the payment of the application

fee as proof of payment for the filing of this petition to form part of this
petition; and
9. That attached hereto, in a separate sheet of paper but a part of this

petition, are three (3) specimens of petitioner’s official signature, true
impression of his dry seal and three (3) passport size colored
photographs.

WHEREFORE, premises considered, it is most respectfully prayed of

this Honorable Court that petitioner be issued a commission as Notary
