AFFIDAVIT OF DISCREPANCY
WE, __________ and ______________, both of legal age, Filipino, and
residents of _________, Zamboanga del Sur, after having been jointly sworn to in accordance with law, do hereby depose and state that :
1. That we personally know TIMOTHY DONTON REYES being our close
friend and neighbor for quite a long time;
2. That in her Certificate of Live Birth her middle name was erroneously recorded as DONTUN while on most of her pertinent documents or specifically on her Scholastic Record Form 138-A and Voter’s Certification her middle name was entered as DONTON ;
3. That the names TIMOTHY DONTON and TIMOTHY DONTUN, refer to
one and the same person.
4. That we are executing this affidavit of discrepancy in order to attest to
the truthfulness of the foregoing facts and for whatever legal intents and purposes this may serve best.
IN TESTIMONY WHEREOF, We have hereunto affixed our signatures this
28th day of March, 2018 at Molave, Zamboanga del Sur, Philippines.
