1.0 INTRODUCTORY STATEMENT

1.1 Introductory Statement


The direction set in this plan applies specifically to The Proposed CONSTRUCTION OF
SNEPCO FACILITY
The general objective of this Plan is to describe how GBEMICH will manage and control the
requirements and objectives for Health, Safety and the Environment on the Project.
GBEMICH SERVICES management will ensure that the HSE requirements are met in the
performance of works, through continually monitoring and controlling, by HSE Management, all
work activities with the aim of reducing the risk of accidents, injuries, illnesses and environmental
incidents associated with these activities, with the Project vision and mind set of being incident
and injury free and providing meaningful HSE performance targets that are measurable and
monitoring progress against these targets for continuous improvement.
The objectives of the plan particularly address the following areas:
 Define the appropriate management structure and processes to be implemented by the project
team and GBEMICH subcontractors during the implementation of the project.
 Establishing and maintaining ongoing HSE programs.
 Establishing and maintaining a structured safety organization.
 Continually emphasizing HSE through ongoing communications.
 Sharing best practice and lessons learned including those from incident and near miss
investigations.
 Providing and ensuring attendance at well-defined HSE induction and training programs.
 Providing appropriate and well maintained personal protective equipment (PPE).
 Tracking HSE performance.

This HSE Management Plan shall, by stating systematic principles and responsibilities, and
referencing procedures and requirements, ensure that all activities are organized and managed in a
consistent manner and in accordance with project requirements and the HSE Policy.
It is a ‘live’ document that requires updating during all stages of the Project / Contract and it shall
be read in conjunction with corporate HSE Plan/Manual and Excavation plan/ Job Hazard
Analysis.
The key information contained within this plan includes:
 HSE responsibilities;
 Induction and training arrangements;
 Various construction activities to be undertaken during the project and their
corresponding associated hazards and the control measures to be applied;
 General strategies of ensuring zero incident on the project.
 Site HSSE rules;
 Incident reporting and management process; and
 Emergency response arrangements.

1.1.0 Location of Site


The project site is located at LADOL FREE ZONE Lagos state, Nigeria.

1.1.1 APPLICABLE STANDARDS/LEGISLATIONS
This Plan has been developed in line with standard best practices on HSE management systems
and in compliance with applicable standards:
 ISO 14001:2004 – Environmental Management Systems – Requirements with
Guidance for Use
 OHSAS 18001:2007 - Occupational Health and Safety Management
Systems Requirements Standard
 International Labour organization Construction code of practice (Code of Practice
on Safety and Health in Construction, approved and published in 1992)
 Relevant Nigeria legislation, standards, guidance notes and other requirements
relating construction may include both not limited to:
 Factories Act, 1987, Cap 126 Vol. VIII P.5459
 Workmen Compensation Act 1987 Cap 470 Vol. XXIV P.16079

1.1.2 GBEMICH SERVICES Management shall demonstrate commitment to the management


of HSE through the following:

 Provision of Adequate Resources, Competent Personnel and Fit For Purpose Equipment
for the execution of the project
 Involvement in and chairing of HSE Meetings.
 Promotion of HSE issues in organization’s communications
 Involvement in HSE inspections, walkthroughs and audits
 STOP work or the work of others if it is unsafe
 TAKE TIME to assess the risks and work safe
 INTERVENE if a hazard is seen
 FOLLOW corporate guidelines and procedures
 PROCEED only if you are TRAINED, PROTECTED, have a PLAN and are in the right
state of Mind.
 REPORT immediately all incidents or near misses and INVESTIGATE causes

1.2 HSE POLICY STATEMENT

1.2.0 HSE POLICY STATEMENT


It is the philosophy of GBEMICH SERVICES that accidents/injuries are preventable and
therefore unacceptable in her operations. GBEMICH SERVICES shall therefore integrate safety
into her business and will vigorously pursue all accident prevention programs through her well-
structured and effective HSE Systems.

GBEMICH SERVICES activities will therefore be organized, planned and executed in such a
manner as to:

- Protect and promote the health of its work force as well as the conduct of its activities in
such a manner not to adversely affect any third party (host Community inclusive).
- Avoid injury to any workers, sub-contractors, and third parties who are either involved in
or are by GBEMICH SERVICES activities.
- Ensure the personal security of the work force and third parties and the security of
property.
- Minimize the impact on the environment in which GBEMICH SERVICES operates.

Every employee of GBEMICH SERVICES shall perform his/her work in accordance with this
policy and work must be suspended when it is believed that essential safety systems are not in
place.

Management and Supervisors will be accountable for the safety of the employees working under
their supervision andwill always be expected to organize and conduct operations in a safe manner.

Employees are reminded that they have a duty under the lay to take reasonable care for their own
safety and safety of others who may be affected by their acts or omissions and also to co-operate
with statutory safety obligations which include adherence to GBEMICH SERVICES HSE Policy,
rules and regulations.

The implementation of this policy is the responsibility of supervisors under delegated control and
co-ordination of HSE Manager.

1.2.1 No Smoking Policy

The purpose of this policy is to reduce fire risk and protect the health of non-smokers. As a general
rule smoking is FORBIDDEN in any non-designated areas and in or public area.
Smoking is prohibited in all of the enclosed areas within the work sites. This includes common
work areas, conference and meeting rooms, kitchen, rest rooms and all other enclosed facilities.
The only designated smoking area is outdoors of the company’s building.
Employees who choose to smoke outdoors of the building to do so at their own risk, No additional
breaks are allowed to any employee who smokes.
Finally smokers and users of tobacco products should dispose the remains in the proper manners.
This is to keep a neat and clean environment for all employees and our visiting client and sub

1.2.1 Drugs and Alcohol Policy

This policy applies to all employees of the company and also intended to apply to employees of
firms doing business with company while on our office premises and project sites.
Gbemich Services is committed to maintain a safe and healthy work place free from unauthorized
alcohol and drugs. We recognize that involvement of drugs or alcohol while working eventually
affects the working efficiency, on our concern is to ensure that employees report to work in a
condition to perform their duties safely and efficiently in the interest of their fellow workers,
customers as well as themselves. In order to achieve this policy the company;
 Prohibits possession of any alcohol or alcoholic beverages and unauthorized drugs within
the company’s office and projects sites.
 Forbids staff to operate any equipment while impaired by drugs or alcohol.
 Requires staff to observe a period of 6 hours abstinence from alcohol prior to reporting for
work.
 Requires staff to report immediate to the supervisor any concerns with regards to the
actions or performance of any other person, which may affect the safety of any other
person on the office premises or project sites.

The management shall support the relevant authorities in the prosecution of the offender, and any
contravention of the above requirements may lead to immediate dismissal from service

1.2.2 Personal Protective Equipment

Use of appropriate Personal Protective Equipment is a mandatory in any work related activity in
our company

It is the responsibility of Gbemich Services to provide, monitor, train and replace all Personal
Protective Equipments to ensure the safety and protection of all our employees.
It is the obligation of employees to appropriately use and maintain their Personal Protective
Equipments in excellent and serviceable condition.
Any defective or worn out Personal Protective Equipment should be reported and attended to for
immediate replacement.
Usage and maintenance of Personal Protective Equipment should conform to the company’s safety
code.

2.0 SCOPE OF WORK

This Health, Safety, and Environment (HSE) Plan defines the overall objectives by GBEMICH
SERVICES for the Project

2.1 Scope of the HSE Plan


This Health, Safety and Environmental Plan has been specifically developed by GBEMICH
SERVICES for the execution of the SNEPCO FACILITY to address systematic incident and
injury free programs in a dynamic Engineering and major construction environment.

GBEMICH SERVICES Project Management Team is the administrator of this plan and shall
support its execution on daily basis. Some of the major activities are:

Although, ultimate responsibility for safety rest with the Managing Director but the safety
requirements stipulated in this plan shall be implemented and maintained by the site Project
Engineer supported by the HSE Officer. The Project Engineer and the HSE Officers are designated
safety critical for the job. Their roles and responsibilities and that of other personnel like
supervisors etc on site are outlined below.

3.2. KEY PERSONNEL AND COMPETENCY

The following personnel are responsible for the implementation of HSE objectives in the execution
of SNEPCO FACILITY.

THE MANAGING DIRECTOR


The Managing Director is charged with the following responsibilities, among which include:-
 Approving articulated HSE policy for the company.
 Evolving new policy and update the existing one in line with our client’s requirements.
 Providing both financial and material resources for the management of HSE affairs.
 Approving incentives for HSE line management.
 Setting HSE targets and ensure strict adherence.
 Championing site audits for enhanced performance and taking effective control against
laxity and inefficiency.
 Chairing site HSE meetings at least twice in six months.
 Establishing a viable communication system in such a way as to be accessible to the least
worker and, or give fair hearing to the needs and problems of his workers.
 Ensuring prompt budgetary approvals without delay.
 Ensuring compliance with statutory and client’s HSE rules and regulations

Site Engineer
 Participating in the preparation and sign-off the safety program prior to commencement of
work.
 Organize sites so that the work is carried out as stated in the HSE Plan.
 Give precise instructions as to the requirements for correct work method.
 Manage interface with Company Site Rep and subcontractors to avoid any confusion about
areas of responsibility.
 Ensure that fit for purpose equipment are available and in use
 Ensure that competent personnel are used for the job
 Ensure that suitable personal protective equipment (PPE) are available and in use.
 Ensure that new employees are properly inducted in precautions to be taken before they are
allowed to start work.
 Obtain permits for all jobs requiring Permit-to-Work

Safety Officer/ First Aider:


 Participating in the preparation and signing off the safety program prior to commencement
of work
 Periodically patrol the work site to verify that the work is carried out under safe conditions,
with no violations of safety requirements.
 Advice the site Supervisors and Workers of corrective action when any unsafe conditions
or violations are observed.
 Check each work procedure from the safety point of view and advise the site supervisors
before commencement of work and, or while working
 Collate, analyses and submit HSE statistics report to Project manager and LADOL site rep
and make recommendations for safety improvement (for incident).
 Maintain and share Learning-From-Incidents (LFIs) and other HSE communications
 Check on the use of all types of personal protective equipment, evaluate effectiveness and
suggest improvements.

The above list is however not exhaustive and shall be updated when new issues are identified
during the project execution.

5.0 PROJECT HSE REQUIREMENTS

5.1 General HSE Requirements


The applicable HSE guideline and requirement among these mentioned below shall be upheld by
the project execution crew.

5.1.1 HSE Rules: The rules will be cascaded during induction to workers and will
be repeated occasionally during safety meetings. The rules will be pasted at the
notice boards around the work environment. Consequence management will apply to
any who disobeys

Working hours: The working hours will be from 8.00am to 5.00pm onsite.

Incident reporting and investigation: The policy of the company on reporting any incident will
be strictly adhered to. All accident/incident will be reported to the client project engineer before 24
hour after the occurrence of the accident/incident. An investigating panel will be constituted
immediately an incident (accident, near miss) is reported to the management.

Stop work authority: The stop work policy of the company will be strictly adhered to. All
workers have the right to stop any unsafe act or condition.

5.2 Safe Work Permits


A permit to work (PTW) is a document signed by an authorized person(s) stating that activities can
be carried out under prescribed precaution. This must be obtained before such operation could
commence. PTW and its complimentary permit (Hot Work Permit, excavation permit, etc.) shall
be obtained revalidated on daily basis and renewed weekly during the project.

5.3 Subcontractor Management


Subcontracting is part of the entire organization plan of GBEMICH SERVICES and a special part
of the contractor execution. GBEMICH SERVICES have a documented policy guideline that
guides and controls the selection and monitors the execution of all subcontracts within the
organization.

5.4 Plants and Equipment Management

The equipment shall be checked daily, before the commencement of any work. The check shall be
done by means of equipment checklist, which will be logged and recorded each day by the project
engineer. Any plant or equipment found faulty shall be removed from site immediately.

5.5 Personal Protective Equipment (PPE)


GBEMICH SERVICES shall provide all employees with relevant PPEs/Safety Equipment to
ensure the safety of their workers. Management will ensure that all her staff will abide by the
GBEMICH SERVICES PPE Policy.

The applicable minimum PPE requirement for this project and the task for which they apply are
listed below:
 Hard Hats
 Safety Goggles
 Ear plugs/muffs
 Nose masks
 Hand gloves
 Work vest
 Coveralls
 Steel-toe Safety bootsEtc.
 Welding apron

5.6 Waste Management:


The types of waste expected to be generated in this project are;
 Wood off cuts

 Food waste

 Plastic waste

 Metal offcut waste

 Cement bags waste


Waste generated from the project shall be latch unto LADOL’s waste management plan.

Color code for waste bin shall be;


 Green -------------------------------- Food waste
 Black -------------------------------- Plastic/paper waste
 Brown -------------------------------- Metal/wooden waste

 Yellow -------------------------------- Hazardous waste
 Red -------------------------------- Combustible waste

The waste will be segregated at source and moved to LADOL designated disposed site.

5.7 Hazardous Materials Management


A SHOC card will be maintained for all chemicals to be used for the project. Workers will be
sensitized on the various hazards associated with the various chemical they may handle in the
course of doing their work at site. The ways of eliminating, minimizing and controlling the
harmful effects of this hazards will be catalogued during JHA review and cascaded down to
worker during HSE meeting, Toolbox meeting and other related avenues.

Ref to SHOC Card manual for the SHOC Cards of the chemicals listed.

5.8 Management of Change


All members of the project team, including the Project sponsors and contractors/vendors shall be
responsible for identification, communication and management of change. The Management of
Change process shall provide the project team with an early warning of conditions that might
affect one or more of the following key areas:
 Reassessment of Project Needs or Requirements (RPNR)
 Equipment
 Materials
 Documentation (Drawings, Plans, Specifications, Procedures)
 Organizational
 Environmental, Social, and Health & Safety
 Cost and/or Schedule
 Logistics
 Security
 Other

The potential changes shall be circulated around the project in an expeditious manner so that a
positive prompt response can be issued to the originator of the change notice.

9.0 HSE COMMUNICATION AND MEETINGS

HSE COMMUNICATION

For staff to be fully abreast on HSE Matters, management has the following mediums with wish HSE issues
are fully communicated throughout the entire operations.

 Weekly Pep which will be held every Monday, and such will be communicated to the employee and
such will last for 10minutes.

 Schedule HSE Meetings

 HSE Signs/notices and Warnings

 Learning From Incidents (LFIs)

 Training program

 Audits/Inspections and Drills

SCHEDULED MONTHLY HSE MEETING

A monthly HSE meeting will be held fourth nightly every month and the meeting shall contain the
following agenda items; this meeting will be facilitated by the HSE department or department responsible
for HSE. This will occur as per Clients expectation and requirements.

 Opening remarks/Prayer (s)

 Safety Moment(s).

 HSE related subject or topic to be designed and chosen by the HSE department.

 Review of Health, Safety and Environment (HSE) Performance for the month: (Review of the content
of the weekly HSE Statistic reports compiled by the HSE Department).

 Review of Incident/Accident (adverse event that occurred).

 Cascade of HSE related Policies.

 Communication of new development such as regulations and other requirements.

EMERGENCY DRILL PLAN FROM OCTOBER 2021-DECEMBER 2022

An emergency drill and tabletop exercise will be carried out on a bi-monthly basic and such will be
coordinated by the department responsible for SHEQ department.

The outcome of the drill shall be recorded and communicated to the participant of the drill. The lessons
learnt will be circulated and a record of the drill kept for reference purpose. The purpose of the
exercise/drill is to test the various emergency plans and to determine if the emergency arrangements are
adequate and fit for purpose.

Weekly toolbox/pep talk meetings will be held on a weekly basis and these meetings will last for
approximately ten minutes. A record of the meeting will be kept with the QHSE department for reference
purpose.

HAZARD AND EFFECT MANAGEMENT PROCESS (HEMP)

GBEMICH SERVICES has carried out general and specific risks assessments for all staff and activities
within GBEMICH SERVICES site and thus attached as Appendix II to this document. The risk assessment
will be in the client format and shall be developed by GBEMICH SERVICES and sent to LADOL SHEQ
Team for review / approval.

An HSE performance target has been set by GBEMICH SERVICES to ensure progressions towards the
long-term goals of no harm to people and no damage to the environment. A performance indicator has been
established and will be monitored and results reported in such a way that can be externally verified

Emergency Response Plans for any unusual event are in place in GBEMICH SERVICES. The
plan tends to highlight what shall be done and persons to be contacted in the event of any
emergency.

Due to the nature of the project, there may be emergencies, which could require contingency
planning, these include but not limited to.
