• Form E of Cargo Ship Safety Equipment Cert. marked nautical chart & ECDIS both provide but only ECDIS provided on board in fact (large navigation paper chart not provide). • The bottom inspection of ship interval exceeds 36 months in Taizhou Port. • ISPP certificate invalid. 012 - Certificate & Documentation - Crew Certificates • Flag endorsement for 4/E not available on board and no endorsement application. • Flag state endorsement for C/E not available on board • Master`s proof of application for endorsement issued by flag state not be shown. 013 - Certificate & Documentation - Documents • Certificate or documentary evidence of financial security for repatriation not be shown onboard. • Certificate or documentary evidence of financial security relating to ship owners’s liability not be shown onboard. 02 - Structural Conditions • During testing the emergency steering gear. The rudder wheel on the stop position but the rudder unable to keep steady. It moved to port side one degree per two second automatically. • Rudder angle unable to keep in order degree (The rudder turned from stbd. To port side automatically) when testing emergency operation in ST/G room. • Two quick closing valves for F.O.T. in E/R lowest floor found leaking. • Alarm of insulation conditions monitoring equipment test out of order. • Bulkhead of E/R near poop deck holed. (ISM) • Starboard door of wheelhouse not keep gas tight. • Close device of cargo compress room ventilation out of order. 03 - Water/Weathertight conditions • PLIMOSOLL MARK NOT IN COMPLIANCE WITH THE LOADLINE CERTIFICATE IN USE. THE COPY OF NON-USE LOADLJNE CERTIFICATE FOUND TOGETHER WITH THE CURRENT CERTIFICATE. • Cleats for C/H coaming missing. • A lot of wedges for No.1 hatch cover starboard side main deck malfunction. • A NUMER OF HATCH COAMING SECURING CLEATS FOUND MISSING OR SEIZED • Air pipes for No.7 WBT & No.1 WBT (TST) rusted seriously, the closing devices of openings for the air pipes seized. • The float of air pipe head of port side FPT missing, the air pipe float of No.3 WBT (S) and APT seized and lacking gaskets, and some air pipe of WBT lacking gasket on main deck. (ISM) • The secured device of air vent on No.4 hatch cover broken seriously. • Coaming of E/R ventilator near poop deck holed. (ISM) • Automatic closing devices of four air pipes to (Fwd/APT, COFF-P & S, Emergency generator FOT) malfunction. • Air pipes of No.3 heeling tanks(S) cannot be closed due to closing device damaged. • Air pipes of No.5 WBT(S) cannot be closed due to closing device damaged. • Two vent. for bosun store on Forecastle corroded with holes. (ISM) • Air pipe top cover gasket for WBT (No.5 DB (P), No.2 WT (P), No.2 DB (P)) not in good condition, not weathertight due to the closing device defective.

04 - Emergency Systems • Shaft of emergency fire pump stuck and unable to be turned during inspection. • Emergency fire pump failed to supply water exceeded 15 minutes after the starting. (ISM) • The crew not familiar with fire drill. (ISM) • Water ingress alarm system in bridge display system alarm. • Water ingress alarm panel in bridge showing system abnormal. • The start battery of emergency generator out of order. • Emergency generator can not be started due to empty start air bottle. • No.1 starting battery of E/G cannot be charge and discharge by the emergency switch board E/G cannot be started by No.1 starting battery. • Following evidence shown the crew not familiar with the enclosed space entry and rescue drill although the drill just carried out on 10th Feb., 2017, crew as injured person not safety entry in the eclosed space parts of equipment as per the drill procedure not took to drill place firstly crew not fa 05 - Radio Communications • MF/HF installation failure due to fire accident. • MF/HF radio unable to transmit and receive in order. • INMARSAT C station failure due to fire accident. • Reserve source of energy for MF/HF installation out of order. • SSB of MF/HF can not automatic start from AC to DC power. (ISM) • MF/HF and nbdp unable to keep working by battery supply. • Second officernot familiar ithoperation of SSB. • 2/OFF not familiar with NBDP operation during inspection. 06 - Cargo opeartions including equipment • Cargo stowage not comply with IMSBC CODE requirements. (Ammonium nitrate based fertilizer stowage in NO.7 Cargo hold directly contact with a metal E/R bulkhead) 07 - Fire Safety • Door between tele exchanger room and wheel house not A-15 class.(ISM) • Cable penetrated the bulkhead between elec. cable trunk and corridor on A-deck, but the penetration did not be sealed properly. • `A-60` class division between Engine room and Steering space where emergency fire pump located in found damage partly, a hole found on the bulkhead. • Pipe penetration with size 300mm x 70mm on deck between bridge-upper and pilot cabin-lower not be sealed, and not meet A-60 fire integrity. • Bottom bulkhead of escape trunk in E/R not arrange with insulation material and part of material made of wood • Fire integrity between E/R and accommodation damaged due to new cable penetrated without insulation. • Fire division between E/R and emergency fire pump station not complied with A-60 requirement. • THE FIRE INTEGRITY OF THE BULKHEAD BETWEEN PIPE & DUCT TRUNK AND CORRIDOR ON A DECK WAS DESTROYED DUE TO SOME ELECTRIC CABLES PENETRATED THIS BULKHEAD FREELY. • Cable penetration from air conditioner room, the cable hole not blocked by thermal insulation materials, and cable also penetrate from E/R to cable trunk and not blocked, many place same. • The self-closing fire door (123) (A-60) of E/R can not be closed tightly. (ISM) • Door access to E/R on funnel deck can not closed tightly. • Fire door access to E/R from corridor on B-Deck can not closed tightly, meanwhile the gasket of the fire door frame aged & broken. • Self-closing fire door (A-60) of the air-lock access to steering gear room from E/R can not be closed. • The fire door access to E/R from changing RM deformed seriously & cannot be closed tightly. • Fire door access to E/R from steering gear room cannot be closed tightly. • Both fire doors of purifier room(protected by CO2 independently) in E/R found the self-closing devices removed and fitted with hold-back hooks • Fire door to ST/Room & fire door of Escape trunk in E/R - Deformed & cannot be closed. Fire door of A/C room cannot be closed tightly due to electric cable penetrated the fire door.

• Self-closing fire door of E/R deformed, broken & cannot be closed. • Fire doors of electric cable space & stair way & galley cannot be closed tightly. • Fire doors access to radio equipment room & bridge on bridge deck cannot be closed tightly. • Door access to E/R from funnel deck cannot be closed tightly & deformed. • Section v/v for local water-base F.E.S. for M/E found leaking seriously and some sprinklers for M/E blocked (Two sprinklers above M/E active other sprinklers not have when tested). • Half of the foam liquid in tank for fixed high expanding foam system (capacity 1200L) (required foam liquid 1200L). • Fire water can not supplied for high expansion foam system. • The remote control for the suction valve of emergency fire pump out of order. • Water leaked seriously from fire main. • Fire main on main deck leaking. • Quick-closing valve on No. 2 A/E fuel inlet line fail to be closed remotely. • Compress air bottle for quick closing v/v in E/R no pressure due to heavily leaking on No.2 group line.(ISM) • THE FIRE DAMPER OF NO.1 E/R VENTALATOR CAN NOT BE CLOSED • The remote quick closing valve of no.3 portside F.O tank out of order • THE QUICK CLOSING V/V FOR NO.2 D.O. SERV. TK WAS KEPT IN OPEN POSITION BY WOOD SUPPORT, AND ENGINEERS UNFAMILIAR WITH THE TESTING OF QUICK CLOSING V/V • THE QUICK CLOSING V/V FOR D.O. SETT. TK OUT OF WORK, CAN NOT FULL STOP FUEL SUPPLY AFTER CLOSE • The quick closing valve for NO.2 D.O.SERV.TK was kept in open position by wood support, and engineers unfamiliar with the testing of quick closing valve. • the quick closing valves valve for D.O.SETT.TK out of work, can not fully stop fuel supply after closing • Quick closing valve of D.O. service tank out of order during inspection. • Fire samper for purifier room suppl air fail to be closed. • THE FIRE DAMPER OF NO.2 E/R VENTALATOR CAN NOT BE CLOSED . • Funnel fire damper fail to be closed • One fire damper of went duck in E/R out of work. (ISM) • Fire dampers of funnel cannot be closed completely. • No.2 ventilation for engine room broken and cannot be closed. • Emergency generator fuel oil leaking arrangement out of order - oil collection device can not be give alarm during PSC inspection. • Jacketed high pressure line oil leakage alarm of No.3 generator engine out of order (oil collecting equipment not fitted) • Fire door and door frame for escape enclosure space in engine room deformed and transmittance, not insulated to the A-60. • One part internal dimensions of engine room escape way only 600mm x 600mm. (ISM) • All engineers unfamiliar with testing of F.O.T. quick closing valve in E/R, and also unfamiliar with the testing of A/E F.O. supply line isolative valve. • QUICK-CLOSING VALVE ON NO3 A/E FUEL LINE FAIL TO BE CLOSED REMOTELY DUE TO AIR LEAKAGE • Quick-closing valves on No.2 & 3 A/E fuel inlet line fail to be closed remotely. • All generator exhaust pipe near turbo charger partly missing thermal insulation materials, about 4 - 6cm in length.

10 - Safety of Navigation • A lot of steps of both sides accommodation ladder combined with pilot ladder broken seriously. • X-Band & S-Band radar failure due to fire accident. • Stern light of navigation light system damaged seriously. • The chart No. 1965, 3888, 3875, 103 for next voyage not provided and the chart no. 103, 1555, 4073 & 4075, 4508 for last voyage not provided. • Port Chart (BA3452, 1760, 1761) not provided onboard and not reported to port state before in port. • Guangzhou port BA chart No.343 expired (last edition 2010)(small correct not be carried out). • VDR reserve battery expired. • Echo sounding device out of function. • Captain failed to ensure appropriate and effective charts used for Qingdao port and approach passage. (ISM) 11 - Life saving appliances • The arrangement of the drainage plug of S/S lifeboat unable to prevent water from entering the lifeboat when lifebot is waterborne. • The engine of port side lifeboat - can not be started by No. 2 battery • Inter-lock unit of on-load hook release mechanism for lifeboat on starboard side - malfunctioned • Port side lifeboat engine can not start by No.2 Battery, starboard side lifeboat engine can not start by No.1 Battery. (ISM) • Lifeboat (S-side) engine failed to start. • The engine of No.1 lifeboat (Rescue boat) failed to start. (ISM) • Doors for both sides lifeboat can not keep watertight. • Foundation of painter release unit for port side lifeboat disconnected with the hull of lifeboat. • DRAIN VALVE FOR P/S LIFEBOAT UNABLE TO PREVENT ENTRY OF WATER AUTOMATICALLY WHEN THE LIFEBOAT IS WATERBOURNE • STBD side lifeboat engine unable to be started by any group battery • Two doors access to port side lifeboat cannot be closed tightly. • The door access to starboard side lifeboat cannot be closed tightly. • Port side lifeboat engine cannot be started by No.2 battery. • The handle of emergency release control in free fall lifeboat was fixed by plastic tape and not workable immediately • Bothside lifeboats cannot started by manually • Port side lifeboat engine failed to start. (ISM) • P & S side lifeboats watertight door has big gap and cannot keep watertight. (ISM) • No. 2 Lifeboat Engine unable to be started • STEERING GEAR OF L/B - MALFUNCTION • Two blocks on top of davit on starboard side lifeboat deck broken seriously. • Emergency launching device (Reserve source) for rescue boat malfunction.(ISM) • Rescue boat engine difficult to start • 2 liferaft (25 persons) and 1 liferaft (6 persons) had not been arranged in place and cannot be immediately used. • The davit-launching type liferaft not avaliable and not meet the requirement of cargo ship safety equipment cert. • The protection covers for travelling block of port side lifeboat davit damaged. Top of port side lifeboat davit broken. • Rescue boat davit hydraulic release system defective (one hydraulic pipe valve rusted and leaked seriously). • All lifebuoys on main deck and lifeboat deck holed & cracked and deteriorated. (ISM) • THE SIDE ROPE OF THE BOTH SIDES EMBARKATION LADDERS AGED SERIOUSLY AND BROKEN. • The side rope for embarkation ladder on lifeboat (S) embarkation deck aged seriously and broken. (ISM) • The side rope for embarkation ladder on lifeboat (P) embarkation deck aged seriously and broken. (ISM)

A lot of steps of embarkation ladder cracked • Embarkation ladders on both sides did not comply with requirement SOLAS 74/CIII&LSA code due to triangular wedges missing. • Stretcher team cannot enter the lifeboat safey at lifeboat platform and the means for holding the lifeboat against to the ship side not be provided. • Gas alarm detection system in CCR malfunction. 13 - Propulsion and auxiliary machinery • F.O. inlet isolate valve unable to be remoteclosed during inspection. • Oil mist detecting system for all D/G (4sets) found malfunction since of 24 Nov. 2015 (each cylinder bore of generator is 330mm, engine capacity 2648kw, RPM720 and E/R at status of the UMS). • The liquid meter of DOST in E/R malfunction, crews connected one plastic pipe instead. (ISM) • The liquid meter of FWT which supply water to local water mist system in steering room malfunction, crews connected one plastic pipe instead. • Cool S.W. inlet and outlet for M/E air cooler water leaking seriously. (ISM) • M/E L.O. cooling S.W. pipe inlet and outlet water leaking several place. • M/E F.W. cooler S.W. cooling pipe inlet and outlet water leaking several place. (ISM) 141 - Pollution prevention - MARPOL Annex I • 15ppm bilge alarm not activate and bilge discharge not automatic stop when sample water cock be closed, which results in oil content meter cannot monitor the effluent from 15ppm bilge separator • Oil water seperator unable to working properly due to 15ppm bilge alarm activate frequently • O.W.S. kept running normally when v/v on sample line closed.(ISM) • Oil discharging solenoid valve of oily water separator out of order. • SAFETY VALVE FOR OILY WATER SEPARATOR OUT OF ORDER • 15ppm bilge alarm not automatic stop when sample water cock be closed, which result oil content meter cannot monitor the effluent from 15ppm bilge separator. • Oil discharging solenoid valve of oily water separator out of order. • No sample water to 15ppm alarm arrangement when OWS running • The oil monitor device of OWS system malfunction. • Bilge pump for O.W.S in E/R can not establish pressure. • Key crewmember was not familiar with the operation of OWS during PSC inspection. 15ppm device always give EE ppm alarm and can not be reset. The oil outlet valve keep on opening position. • No alarming activated and O.W.S. kept on running when sample water cut off. • 15ppm alarm not raised when sample water flow through it and 3-way valve not active. • The truly representative sample of effluent with an adequate pressure and flow not provided for 15ppm bilge alarm due to no sample water passed through oil content meter during OWS working • 15PPM bilge alarm device out of work when inspection bilge sample water not supplied but OWS still running. Clean and sample water not flowed from outlet pipe of 15PPM alarm device.

144 - Pollution prevention - MARPOL Annex IV • Sewage treatment plant malfunction, the activated return pipe were painted, the bulkhead between aeration chamber & settling chamber inside S.T.P rusted seriously & broken. • THE STERILIZATION FOR SEWAGE TREATMENT PLANT NOT SATISFACTORY. AND THE VISUAL GLASS INDISTINGUISHABLE • SEWAGE TREATMENT PLANT BOTTOM BYPASS V/V KEPT IN OPEN POSITION AND UNTREATED SEWAGE DISCHARGED OVERBOARD • Sewage treatment plant bottom bypass valve kept in open condition and untreated sewage discharged overboard. • Tablets of sterilization for sewage treatment plant expired on April 2015. • Sewage water overboard directly. • Untreated sewage discharge overboard via grey water pipe directing to overboard pipe. • Untreated sewage discharger directly overboard when vessel entered China’s territorial sea from around 07:00 17 May 2017 to 09:55 17 May 2017. (ISM) 146 - Pollution prevention - MARPOL Annex VI • Incinerator unable to start. 15 - ISM • The ships SMS failed to ensure the effective implementation of element 6 of the ISM code as evidenced by the crew unable to communicate under emergency situation in working language. • All deck officers not familiar with essential shipboard operational procedure relating to cancelling false distress alert (VHF DSC & MH/HF DSC, INMARSAT-C & etc.). • Responsible officer not familiar with essential shipboard operation procedure relating to echo sounder. • Responsible officers (C/OFF, 2/OFF, 3/OFF) not familiar with MF/HF equipment operation. • SMS implementation on board not satisfied due to the key shipboard operations concerning environment protection required by MARPOL 73/78 as evidenced by deficiency marked (ISM). • Ships SMS implementation not satisfied due to ships maintenance regarding to item no.4 / 5/ 10 additional audit should be carried out • SMS as implemented on board cannot ensure the safety operation carried out effectively as evidence that the permission of entry into enclosed confined space not issued, but E/R crews overhauled M/E cylinder no2. on 18th JAN, overhauled M/E cylinder No.4 on JAN 2017 measured M/E Crankshaft deflectio • Chinese port chart No.1429 was send on board 8 months ago, and was in using but no any Chinese notice to mariners supplied to ship and the chart never be updated by the ship`s officer. • The SMS as implemented on board failed to ensure effective emergency preparedness as evidence by deficiencies marked (ISM). • SMS as implemented on board cannot ensure the emergency preparedness as evidence deficiencies marked (ISM). • Deficiencies marked (ISM) are objective evidence of a serious failure, or lack of effectiveness, of implementation the ISM Code. • The deficiencies marked (ISM) are shows ship SMS running failure, additional audit should be carried out. • The deficiencies above show ship SMS running failure, additional audit should be carried out before departure. • In the view of above deficiencies marked (ISM), maintenance of the ship and equipment cannot be carried out as per the requirement of ISM Code. • SMS implemented onboard failed to ensure appropriate and effective maintenance the ship and equipment as evidenced by deficiencies marked (ISM). • In the view of the deficiencies, the maintenance of the ship and equipment cannot be carried out as per the requirement of ISM and ship’s SMS. • In the view of above deficiencies, the maintenance of ship and equipment cannot be carried out as per ISM code and SMS, so we recommendation that additional audit will be carried out. • The above deficiencies showing that ship's SMS failed to ensure ship and ship's essencial equipment in good maintenance.

• THE ISM SYSTEM, AS IMPLEMENTED, DOES NOT ENSURE THE SHIPBOARD OPERATIONS AND MAINTENANCE COMPLY WITH SMS REQUIREMENTS, AS EVIDENCED BY THE ABOVE DEFICIENCIES • All deficiencies are objective evidence of a failure or ineffectiveness of implementation of the ISM Code. • No.4 and No.5 hatch cover holed. • Deficiencies marked (ISM) are objective evidence of a serious failure, or lack of effectiveness, of implementation the ISM Code (Parag. 10). • SMS implemented onboard failed to ensure appropriate and effective maintenance of the ship and equipment onboard as evidenced by deficiencies marked (ISM). • The SMS as implemented on board failed to ensure effective emergency preparedness and ship maintenance as evidenced by dificiencies no.1, 4, 6, 8, 9, 10 & 11 above mentioned 16 - Security • Access to ship not controlled strictly strictly (during PSCO coming onboard, no gangway watch provided.) 17 - Other • THE CONDITION OF THE SHIP AND ITS EQUIPMENT (DEFICIENCY CODE 17) NOT MAINTAINED TO CONFORM WITH THE PROVISIONS AS PER 5OLAS I-REG11. 181 - Labour Conditions - Minimum requirements for sefarers • One seafarer's medical cerificate expirated 182 - Labour Conditions - Conditions of employments • Crew have not been paid onboard wages since 1st July 2017. 183 - Labour Conditions - Accommodation, recreational facilities, food and catering • Insufficient fresh provisions and dry stores onboard for intended voyage. 184 - Labour Conditions - Health protection, medical care, social security • The ship exemption certificate of fixed fire extinguishing arrangement in cargo spaces attached to SC Certificate expired. • Gas detector measurement not carried out in the accommodation during fumigating this voyage. • Gas detector measurement not carried out on deck during fumigating this voyage.

07 - Fire Safety • No portable fire extinguisher stowed near the entrance of electric locker & navigation locker. • The heat insulation around the fire door to steering gear room form E/R broken.
