Mobile Application User Guide

Android and iOS version 2.0


Introduction

We’re about people and developing the SmilePh app to cater your
needs in one application is our way to bring us closer where you
could easily check your Policy Details, request a Letter of
Guarantee, contact or find the nearest Providers and Doctors, view
your Claims History, and send an Inquiry. Get the most out of the
app by downloading the latest version where you can now easily
file your reimbursement claims through the app.

PART 1: Getting Started

1. Downloading the app

1.1 Android - App Installation

Step-by-step guide
This shows you how to install Medical Smile App from the Play Store on your Android
device.

• From the Home screen, navigate: Apps icon > Play Store. Installation requires
atleast Android version 8.1 or above.
• If unavailable, swipe up from the center of the display then tap Play Store.
• To download the Medical Smile App, tap the Google Play search bar (at the top)
then enter the app name or tap the Microphone icon to speak the app name.
• If you can’t find the app, visit:
https://play.google.com/store/apps/details?id=ph.com.av_asian_life

• Select an app then tap Install.


• To continue, review the required app permissions then tap Accept.
1.2 IOS Installation

Step-by-step guide
• From a Home screen, tap App Store . Installation requires atleast IOS 13 or above.
• To install apps, you must sign in with your Apple ID or create one.
• To browse the App Store, tap Apps (at the bottom).
• To search for the Medical Smile App, tap Search (at the bottom) then enter the
name of the app.
• If you can’t find the app, visit:
• https://apps.apple.com/ph/app/smile-ph/id1488302294
• Tap the app.
• Tap GET then tap INSTALL.
• If you have a compatible iPhone with Face ID enabled, double-tap the Side button
then look at the screen to authenticate.
• If prompted, sign in to the iTunes Store to complete the install.
PART 2: Your Account

1. Signing Up
• Tap Sign Up button on the App’s
Login Screen.

• Enter your card number and fill all


the required information. Remember
to provide your correct birth date
and you must be 18 years old or
above. Also, the user is required to
use their corporate email to register
successfully. Dependents can also
register the same way.

• Once all correct information has


been filled, tap Create account
• Read and Agree to the Data Privacy
Policy and Terms and Conditions of
use.

• Choose how you want to


verify. For users with email
enforced policies, you may
only verify via your corporate
email.
• Type the code you received thru
email. (if unable to receive a
verification code. Tap resend code)
• Tap verify.
• You’re registered!

2. Forget Password

• Tap the information icon at the upper


right corner of the screen.

• Tap “Forgot Password” button.

• Enter the email you used for
registration, then tap “Submit”.

• Choose how you want to


verify. For users with email
enforced policies, you may
only verify via your corporate
email.

• Type the code you received thru


email. (if unable to receive a
verification code. Tap resend code)
• Tap verify.

• Type your new password


• Re-Type
• Tap submit
• Password change success!

3. Login
• Type your registered username and
password.
• Or use your device
Fingerprint/Facial recognition
functionality.
• Tap Login

• Success! You’re logged in!

4. Edit Profile
• Type your registered username and
password.
• Tap Login

• Tap the my profile icon

• Tap the edit icon to edit profile.

5. Edit Contact Info

• Tap the edit icon to edit contact info.


Tapping on mobile no. will be verified
via email, and vice versa.

• Toggling these switch buttons will


enable/disable the dependents to
fille a claim or request LOG.

• Choose how you want to


verify. For users with email
enforced policies, you may
only verify via your corporate
email.

5. Edit Contact Info

New Mobile no. • Type your new mobile no./email then


tap “Submit”.
Re-type new Mobile no.

• You have successfully updated you


contact info!

6. Policy Details
• Type your registered username and
password.
• Tap Login

• Tap the Policy details bar

• View Policy details screen.

• Tap Dependents to see dependent

PART 3: Find Doctor/Provider

1. Find Provider
• From your Home Screen >
• Tap Find Provider.

• Search Provider using the ff:


• Nearby( look for a Hospital,
Clinic, or Both near you)
• Advance Search to filter by(
Type, Province, City)
• Type Search

• On the results you can tap the ff:


• Get Directions>>
• Redirect to Google Map
• Call Now>>
• To contact the Provider
PART 3: Find Doctor/Provider

2. Find Doctor
• From your Home Screen >
• Tap Find Doctor.

• Search Doctor using the ff:


• Nearby( look for a Doctor near
you)
• Advance Search to filter by(
Specialty, Province, City)
• Type Search

• On the results you can tap the ff:


• Get Directions>>
• Redirect to Google Map
• Call Now>>
• To contact the Doctor’s
Clinic/Hospital
• View all>>
• To view all the doctor’s
affiliated Clinic/Hospital.
PART 4: Letter of Guarantee (LOG)

1. View LOG History (In-Patient and Out-Patient)


• From your Home Screen >
• Tap Letter of Guarantee.

• Swipe right to view In-Patient LOG

• Swipe left to view Out-Patient LOG

2. Request LOG

Tap Request LOG.

Provide all the necessary


details then tap “Submit”.

• Choose how you want to


verify. For users with email
enforced policies, you may
only verify via your corporate
email.

• Key in the OTP you received via


your email then tap Verify.

• You have successfully posted a


LOG Request!
PART 5: Claims

1. Claims History
• From the Home Screen
• Tap Claims bar

• You can view all your filed


claims here.

2. File a Claim
• To file a claim, the member
should download a claim form
first.

• Select claim type and patient


then tap ‘Submit’.

• Tap ‘’Allow.

• Tap the download button at


the upper right corner to save
the pdf claim form and sign it.
You will need to upload the
signed version of this claim
form later.

• To file a claim, tap ‘File a Claim’


button (available to
members/employees only).

• Fill all the required


information. Attach all the
required files in jpg/jpeg
format including the signed
claim form earlier.

• Choose how you want to


verify. For users with email
enforced policies, you may
only verify via your corporate
email.

• Key in the OTP you received via


your email then tap Verify.

• After successfully posting your


claim, you will receive a SMS
regarding the receipt of your
claim.

3. Cancel a Claim

• In your claims history, tap the


triple dot icon.

• Tap on ‘Cancel Claim Request’.

• Tap on ‘Yes’ if you want to


confirm, ‘No’ if not.

• You have successfully cancelled a


claim.

4. View Computation Form

• Tap on ’View Computation Form’.

• You can save a pdf copy of


computation form by tapping on
the download icon.
PART 6: Inquiry

1. Submit an Inquiry
• From the Home Screen
• Tap Inquiry

• Select the subject of your


inquiry from the drop-down
menu and type the body of
your inquiry.
• Tap Submit.

1. Set Quick View ON or OFF


• Type your registered username and
password.
• Tap Login

• Tap the my profile icon.

• Tap “My Profile”.

• Toggle the
Quickview switch
button.

1. View FAQ

• Tap the information icon at the upper


right corner of the login screen.

• Tap “FAQ” button.

• Tap any of the questions


and their corresponding
answers will appear in a
drop-down.
PART 9: Compatibility

This app is compatible with Android and iOS devices with a minimum
operating system (OS) requirement of Android 8.1 and iOS 13.

Note:
Users won’t be able to update or install the latest version if the device does not
meet the required OS (please see below sample).
