This document is a skills checklist for a Physical Education course assessing a student's performance in the Prelim Period. It lists performance standards in fitness conditioning and provides space for the instructor to initially rate the student's overall level based on a 4-point scale, with 4 being the highest. The instructor can then note their general observations, recommendations, and sign off with their assessment date. The student also signs to confirm receiving the evaluation. The checklist is used to holistically evaluate the student's understanding of course concepts and competency in physical fitness skills.
