    I Jerick Bongcales Tapayan, of Legal age, Filipino, single and resident of Sitio Monica,
    San Ricardo, Babatngon, Leyte and Jibby Bongcales Tapayan, of legal age, Filipino,
    single and resident of Blk 8 Lot 36 Jeremiah St. Adelina 1 San Antonio, San Pedro
    Laguna do hereby appoint Alecziz Tapayan Cas, of legal age, Filipino, single and
    resident of Blk 8 Lot 36 Jeremiah St. Adelina 1 San Antonio, San Pedro Laguna as our
    lawful Attorney-in-fact for us and our behalf to do and perform the following acts and
    deeds.

    To file, transact, sign and represent us and execute any instrument or documents which
    may be needed or required in connection with our legal claims from Social Security
    System, Pag-IBIG and the like, as legal beneficiaries of Alicia Bongcales Tapayan,
    our mother who recently passed away on May 22, 2023. Likewise our Attorney-in-fact is
    also authorized to file, transact, sign and receive any benefits from her employer JayJay
    Apparel located at San Miguel compound, Amante St., Brgy Landayan, San Pedro, City
    of Laguna.

    To receive and sign in our behalf, all payments representing our legal benefits made by
    the aforementioned relevant government agencies and Jayjay Apparel.

    HEREBY GIVING AND GRANTING unto said Attorney-in-fact to do all acts, deeds,
    matters and things whatsoever in or about the legal claims as legal beneficiaries as we
    could legally or lawfully do in our own proper person or personally present for purposes
    of completing the transactions described herein and giving our Attorney-in-fact full
    power and authority to do and perform all and every act and deed.
