Please answer all the following performance factors to the best of your abilities. Place a mark in the appropriate
box to indicate your rating. Once you have finished the rating, total all of the scores in each of the columns. The
grand total is your evaluation rating. Staff to complete Part A before the appraisal interview and return it to the
appraiser.
INSTRUCTIONS TO APPRAISER: *Rater must read instructions to appraiser before rating.
1. Keep an open mind while assessing an employee. Do not let anger or personal grudges rule your
judgement.
2. Base your judgement on the requirements of the job and his/her performance, rather than
comparing to others doing similar work.
3. Be fair. Overly generous or unduly strict assessments may harm an employee’s career.
4. Keep in mind that your assessment is based on a 6-months period of review. Do not be affected by
any recent performance or any particular/isolated incident.
5. Consider one factor at a time and make comments where necessary to provide additional
information.
6. Upon completion, review and check your rating.
