Your appointment for a Job Capacity Assessment

We are writing to you about your claim for Disability Support Pension.

We have made an appointment for you, so we can assess how your disability or medical
conditions affect your ability to work or participate in other activities. We will also discuss services
that may help you to improve your work capacity and refer you to a suitable employment services
provider if this is appropriate for you.

During your appointment, your assessor may talk to you about:


• any medical conditions or barriers you have
• details of any recent employment assistance or rehabilitation you have had
• a referral to services to help you find and keep a job.

Calls from us will be from a private number. Please allow up to 1 hour for your appointment. The
actual time needed will depend on your circumstances.

You may have a relative, friend or support person with you.

Important information
If you have medical information that you think is relevant to this assessment and you have not
given it to us previously, please provide it before your appointment. You can do this by:
• going to my.gov.au and signing into your Centrelink online account. You can upload your
documents (up to 5 MB per file) by selecting ‘Upload documents’ from the home page.
• using the Express Plus Centrelink mobile app and selecting ‘Upload documents’. You can
download the free app to your mobile device. Make sure you have the latest version installed.

claim for Disability Support Pension may be rejected.

We have referred you for a Job Capacity Assessment.

How your appointment will be completed


An assessor, who is a qualified health or allied health professional will complete your appointment. Please bring
any new medical evidence you have not already given us to your assessment. There is no charge for this
service.

Your assessor will complete a report with the information you have provided about your ability to work and may
refer you to an employment services program. This may include one of the following:
• Workforce Australia
• Disability Employment Services
• the Community Development Program, if you live in a remote area.

Before you attend this appointment you can find out more about employment services, including providers in
your area and what services they offer. You can do this by going to:
• workforceaustralia.gov.au or call 1800 805 260 for more information about employment services
• jobaccess.gov.au or call 1800 464 800 to talk with a JobAccess Advisor about Disability Employment
Services.

Once your assessor determines your employment services program, we will ask you to choose a specific
provider for referral.

Assessors are required by law to protect your privacy and provide a confidential service.
