Me: "Hello, I'm Kian Justin Hidalgo, your student nurse from Saint Paul University Philippines.
Goodafternoon and thank you for coming in today. Before we start this discussion I would like to
ask first if saan po kayo mas comfortable na language na gamitin ko, do you want me to speak in
English or in tagalog?
Me: Okay. If you need clarification sa mga questions na itatanong natin, feel free to ask again.
And also, you may use whatever language you like.
Me: So the purpose po of this session is to better understand po yung pinagdadaanan nyo and in
way po namin kayo matutulungan. Your comfort and privacy are a priority, and everything we
discuss will be kept confidential. If you ever feel uncomfortable or need a break, please let me
know."
Me: So to start, can you tell me a bit about yourself? Your name, age, occupation, and kung saan
po kayo nakatira?
I: I’m Elio Batungbakal from Maria Ramos, Bulacan. I’m 21 years old, and I’m currently a
Tourism student from De La Salle University.
Me: What brings you here today? Can you describe the main concerns or symptoms that have
been bothering you?
I: So lately I’ve been having severe episodes of anxiety lately. It’s affecting my performance at
school. Hindi ako makapagisip ng maayos and I feel like I don’t what to do. I worry too much to
the point that I can’t sleep properly. I worry too much about everything. And I feel like I can’t
control it. I feel like my thoughts are bigger than myself.
Me: I'd like to understand your experience of anxiety better. Can you tell me about any specific
symptoms you've been having, like restlessness, muscle tension, or irritability, aside from
excessive worry and sleep difficulties?
I: Well, I’ve been having this pain and stiffness all around my body, I get easily annoyed or
overwhelmed about the simplest thing, and I always feel exhausted even after sleeping or resting.
Me: How long have you been experiencing these symptoms, and how severe do you feel they are
on a scale from 1 to 10, with 10 being the most severe?
I: I’m not really sure if when, pero mga nasa months na den. And on a scale from 1 to 10, mga
nasa 9.
Me: Are there specific situations or triggers that seem to make your anxiety worse? Do you
notice any patterns to when these symptoms occur?
I: Kapag nagaaway po kami ng partner ko at ng family. Yung symptoms I feel like mas worse
sya sa gabi, or kapag magisa ako, pero it could happen talaga anytime. Pwedeng habang
naglilinis ako, gumagawa ng school works, or I’m out with my family or friends. Basta if there is
something na nagtitrigger like negative thoughts or flashback, dun sya nagproprogress na maging
worse.
Me: How has anxiety been affecting your daily life, such as work, relationships, or other
activities you enjoy?
I: It affects me really bad to the point na hindi ako nakakafunction ng maayos araw-araw. Paki
ramdam ko wala akong gana like I rather stay at home and machining ng mga kanta through my
earphones or speaker.
Me: Let's talk a bit about your life. Have there been any significant life events, stressors, or
changes that you believe might be related to your anxiety symptoms?
I: Nagaway kami ng family ko, iniwan ako ng partner ko, hindi na kami okay ng mga bestfriends
ko. Other than that, I feel like malaking factor din yung nangyaring bullying saakin noon
wherein everyone was laughing at me habang nasa spotlight and nakatayo ng magisa sa harapan
nila. That time I started hating myself too.
Me: Have you ever received treatment or therapy for anxiety in the past? What coping strategies
have you tried, and how effective were they?
I: Meron naman na before which is yung CBT, kaso di ako gaanong makaattend sa sessions ko
nun eh I think due to my busy schedules na den.
Me: Do you have any medical conditions or take any medications that might be relevant to your
anxiety?
Me: I need to ask, have you ever had thoughts of hurting yourself or others or any recent suicidal
thoughts? Your safety is important, and we can provide help if needed.
Me: Are there any other mental health symptoms or concerns you'd like to share? Symptoms like
depression, panic attacks, or obsessive thoughts are not uncommon alongside anxiety?
I: I get panic attacks when my thoughts and worry become uncontrollable.
Me: Okay. Thank you for sharing this information with me today. It's a crucial step toward
understanding and managing your anxiety. In our next sessions, we'll work together to develop a
