resident of P-18 B2 L9, Princess Homes 5, San Vicente, Butuan City, under oath, hereby depose and state that:
1. I am the daughter of CONRADO SARIO LABAJO JR. and MARIA
LIDIA RICONALLA LIBRE; 2. I am the one processing the application for my parents’ Late Registration of Birth due to their age and my mother was the one taking care of my father who currently suffering mild stroke; 3. I am executing this affidavit to attest to the veracity of the foregoing and to support the application for Late Registration of Birth, CONRADO SARIO LABAJO JR. and MARIA LIDIA RICONALLA LIBRE.
