Incident Involving Hatch and Watertight Door

Background

In a harbor a cargo vessel was getting ready to unload its cargo. The crew had opened
the hatch covers. Watertight doors to reach the cargo holds. Unfortunately due to a mix
up and oversight one of the watertight doors remained improperly closed and secured.

Incident

As the unloading commenced rough seas caused the ship to sway heavily.
Consequently water seeped into the cargo hold through the open watertight door
resulting in flooding within the hold. This flooding inflicted damage on the cargo. Posed
a threat to the ship's stability.
