I, DARYL M. CATADA, of legal age, Filipino, single and a resident of

Purok 1, Tigatto Relocation, Davao City, Philippines, after having been duly

1. I am the same person who caused the execution of this affidavit;
2. I was a company driver before at a Surplus Company, located at

McArthur Highway, Toril Proper, Davao City from November 2018 to
March 2023;
3. On March 2023 while I was driving along Dumoy, Toril road, A Police
Officer apprehended me and the same I was issued a Temporary
Operator’s Permit for a violation of defective light and confiscated my
Driver’s License bearing ID. No.:K14-19-002175, also that was my
last day on the above-mentioned company;
4. The Land Transportation Office (LTO) requires me to submit an

photocopy of the Official Receipt and Certificate of Registration
(OR/CR) of the company vehicle I was driving that time for me to claim
my driver’s license. However, I couldn’t provide such document for the
reason that the Surplus Company has already closed due to
bankruptcy;
5. I am requesting the Land Transportation Office (LTO) of Davao City to

consider my request and to release my Driver’s License, after I paid the
amount for the said violation ;
6. I am executing this affidavit of explanation to attest to the truth and to

inform the proper authorities and for whatever legal purpose this may
serve.
