ULNAR NERVE

Ulnar nerve(C8,T1),terminal branch of medial cord of
brachial plexus , passes medial to brachial artery in
arm , without supplying any structure.

8cms above intercondylar line of humerus , it pierces


inter muscular septum and runs in posterior
compartment along the anterior border of medial head
of triceps.
 ARCADE OF STUTHER : It is a canal through which Ulnar nerve
passes, formed by fascia running from medial head of triceps to inter
muscular septum .

 Then it runs behind the medial epicondyle , it pierces Osborne's


fascia and enter the cubital tunnel .

 Later it runs between the 2 heads of FCU and enter the anterior
compartment ,supply 2 muscles in forearm – FCU (flexor carpi
ulnaris) and MEDIAL HALF OF FDP (flexor digitorum profundus).

 4cms above wrist joint it gives dorsal cutaneous branch , at pisiform


bone pierces deep fascia and enters the posterior compartment and
supplies sensation over dorsum of hand and wrist joint .

 At wrist joint it passes superficial to flexor retinaculum.


 GUYONS CANAL :
 ROOF – VOLAR CARPAL LIGAMENT
 BASE – TRANSVERSE CARPAL LIGAMENT
 MEDIAL – PISISFORM
 LATERAL- HAMATE

 After passing through this canal, Ulnar nerve divides into superficial
and deep branches

 Superficial branch supplies sensation to medial 1 ½ fingers


 Deep branch supplies to -
1) Hypothenar muscles (flexor digiti minimi, opponens
digiti minimi, abductor digiti minimi)
2) Medial 2 Lumbricals
3) All Interossei muscles
4)Adductor pollicis
INJURIES OF ULNAR NERVE
Ulnar nerve entrapment can occur at your elbow or
wrist. Types include:
Cubital tunnel syndrome: Pressure or pulling and
stretching of your Ulnar nerve in your elbow region.
Guyon’s canal syndrome: Pressure on your Ulnar
nerve in your wrist region.
 Ganglion cysts are noncancerous fluid-filled sacs that form near your
wrist joint. These cysts cause up to 40% of Guyon’s canal syndrome.
Another 45% of Guyon’s canal syndrome diagnoses occur for no
apparent reason (idiopathic). Less commonly, cysts can also cause
pressure on the Ulnar nerve within your elbow.
Other causes of Ulnar nerve entrapment and damage
include:
 Accidents and trauma.
 Arthritis.
 Broken bones and bone spurs.
 Complications from surgeries to treat carpal tunnel or
shoulder fractures.
 Diabetes.
 Dislocated elbow.
 Pressure from casts or splints or using crutches.
 Tumours or cysts.
FINDINGS
CLAWING OF MEDIAL 2 FINGERS - paralysis of
medial 2 lumbricals , causing extension of MCP and
flexion of IP JOINTS .
