    Kalubihan Mountain View, Talamban Cebu, Cebu City, Philippines, after
    having been duly sworn in accordance with law, hereby deposes and states
    that:

    1. I am CHARMAINE FAYE BONTUYAN;

    2. In my Voter Certification, my first name is CHARMAINEFAYE, one


    word when it should have been two words;

    3. I am executing this Affidavit and attaching herein my PSA to prove


    that there is a clerical error in encoding my name in the Voter
    Certification with VIN 2217-1031A-L3094C.B20000-5;

    4. I am seeking the correction of my first name in the Voter


    Certification with VIN 2217-1031A-L3094C.B20000-5;

    5. I am executing this Affidavit to attest to the truth of the foregoing


    statements and to support my claim to correct the erroneous entry
